We have the opportunity to recruit two Advanced Clinical Practitioners for HCPC/RGN to our GI Faster Diagnosis team who will be responsible for clinical triage and review of GI patients referred into the 2 week suspected cancer pathway. The post holder will be leading and supporting pathway improvement work expediting clinical triage and streamlining the faster diagnosis pathways in line with the Faster Diagnosis Framework and Best Practice Timed Pathway guidance.
The Faster Diagnostic Framework aims to deliver:
- an earlier and faster diagnosis for patients, whether or not they are diagnosed with cancer
- excellent patient experience, a holistic assessment of patient needs, and streamlined support across community, primary and secondary care
- increased capacity in the system, through more efficient diagnostic pathways
- support to healthcare providers to reach the Faster Diagnosis Standard.
Two ACP roles available to support the GI FDS pathway to work alongside a consultant nurse, matron and Band 7 CNS team. The base signed to be agreed at interview with some cross site working.

The successful applicant will be required to work autonomously at a ACP level working closely with our colorectal nurse consultant and wider team within the cancer services and other directorates to provide the highest level of support for patients.
You will have a clear understanding of the surgical nursing, cancer pathways, and be committed to improving patient experience. You will also be committed to the development of advanced clinical practitioner role and demonstrate evidence of on-going professional and academic development. A successful appointment will depend upon interview and fulfilling service requirements.
We are a busy directorate with varying demands and priorities, it is essential you are able to be responsive to the changes and challenges, working autonomously with competing demands.

Patient Care Delivery
- Working autonomously - using an advanced level of specialized clinical expertise and judgment in assessment, planning and evaluating care.
- Communicate effectively with patients and their families in order to assess needs and to provide relevant support, information, education, advice and counselling.
- Ensure all patients have access to written and verbal information
- Attend relevant network meetings
- Collaborate with and refer to other relevant allied health professionals and medical colleagues as required.
- Request appropriate investigations such as CT scans. Endoscopic tests, blood tests, and act on results appropriately.
- Initiate admission to secondary care as appropriate.
- Be the main source of support for junior faster diagnosis nurses in delivering the consultant led, nurse delivered faster diagnosis pathway.
- Offer expert opinion to colleagues and patients in secondary and primary care.
- Run nurse-led physical and virtual clinics taking full medical history, carrying out clinical examinations including abdominal and digital rectal examinations, assessing disease activity, making recommendations for treatment.
- Attend and proactively participate in relevant specialist nurse forums at Trust, Network and national level
- Effectively liaise with consultant and other members of the multi professional team to achieve a coordinated specialist service.
