Job overview

Sutton Health and Care is an innovative partnership of providers working across Sutton (GP Federation; Local Authority; Mental Health Trust and Acute Trust) to provide integrated services for the local community. Main duties of the job

Band 7 Nurse Practitioner

We are looking for an enthusiastic person to work as part of the Sutton Health and Care At Home service. You will be part of the MDT team that helps to prevent admission from community, accelerate discharge from emergency department and acute medical wards and provide supported discharge through discharge to assess pathway.  You will be particularly supporting the front door Frailty hub as a part of Sutton Health and Care  At Home Team focussing on further development and smoot hrunning of the frailty pathway. We are innovative and people focused with a 'one team' ethos, which means better quality and more joined up care for our patients and families.   The team strives to constantly improve service delivery and contributions from all staff are highly valued.  We will provide professional development opportunities and support you to provide great care to our patients.

Education/Qualification

Essential criteria
  • Registered Nurse with current NMC registration
  • Specialist practitioner qualification/Advanced Practitioner/ or working towards
  • Evidence of CPD
  • Mentorship or equivalent course
  • Non-medical prescribing or working towards
Desirable criteria
  • Specialist Practice Teacher

Experience

Essential criteria
  • MDT working
  • Experience in managing complex cases
  • Experience in utilising Clinical examination and diagnostic reasoning skills
  • Experience of conflict management
  • Experience of mentoring students
  • Experience of care planning
Desirable criteria
  • Experience of undertaking clinical audits
  • Experience of contributing towards policies/procedures

Skills

Essential criteria
  • Effective/Holistic Clinical assessment and reasoning skills
  • Excellent communications skills and able to work unsupervised or with minimal support
  • Ability to demonstrate and use initiative, work under pressure, balance multiple priorities and meet deadlines
  • Insight into current issues relating to delivery of community/primary care , Understanding of the CQC and policies and current trends in health and social care
