Do you want to work for a forward thinking organisation? An exciting opportunity has arisen as part of the ongoing development of the All Age Intensive Support team. We are looking a Band 3 Clinical Support Worker to join the  part of the team specialising in Learning Disabilities.

We are looking for an enthusiastic and dedicated Clinical Support worker who has experience of delivering high quality, person centred services with the aim of achieving positive life outcomes for individuals. In conjunction with a MDT you will deliver a high quality service aimed to provide specialist intervention with a view to providing care within the person’s home. 

The IST forms part of the wider Black Country offer supporting individuals with learning disabilities, mental health problems and Autism who are experiencing difficulties which may otherwise lead to placement breakdown or hospital admission. As such the team is reliant upon the interface between services within and outside of Black Country Trust.

This is a challenging but rewarding role and we encourage applications from experienced clinical support workers as we believe having a diverse workforce will lead to better outcomes for our service users. Experience in working with people diagnosed with a learning disability people is  essential to apply, we do encourage applications from individuals who are flexible, positive and open to exploring alternative approaches.

Prif ddyletswyddau'r swydd

  • To assist in assessment of the client as support to and under the supervision of, a registered nurse or allied health professional staff.
  • To follow individualised care and therapeutic programmes for patients under the instruction of the nursing staff and allied health professionals.
  • To contribute to the clients plan of treatment by communicating and documenting any observations of patients behaviours and/or physical health concerns to the nursing staff/team/MDT.
  • To promote communication and act as an advocate for patients who are experiencing difficulty in communicating.
  • To work with all members of the multi-disciplinary team to promote quality care.
  • Work in collaboration with the client, involving them at all times with their care.
  • To ensure, by attitude, approach and response, the continuation of good working relationships between staff, clients, families and care providers.
  • To travel to conduct community visits across four localities within the trust.
  • To develop and build skills and knowledge in the area of Autism and mental Health by undertaking relevant training.
  • Ability to work autonomously/Lone working in line with the trust policy.
  • To attend monthly supervision and to complete and attend annual Personal Development Conversation.
  • To be responsible for maintaining accurate records and other documentation  
  • To work flexibly & to undertake any other duties of a similar nature consistent with the responsibilities of this post in order to provide a quality IST service.

As the post involves working across the 4 Black Country areas having a full driving licence and car is essential to the role. The team operates an 8-8 service so flexible working is necessary between these hours as dictated by the needs of the service and its users. This includes Bank Holidays and weekends.
