Senior Community Healthcare Assistant - Apprenticeship Opportunity

Job overview
The Healthcare Assistant will contribute to the assessment, implementation and delivery of high quality patient care within a professional team. Will demonstrate responsibility to patients, relatives and colleagues through contribution to the development and monitoring of agreed evidence based practice and to the health and social care programmes.
The Planned community nursing team operates 7 days per week between the hours of 8.30 and 17.00.
This is a developmental role with the opportunity to progress from a Band 2 to Band 3 upon successfully obtaining the relevant qualifications.
If you don't already have the relevant Level 2 (for band 2) or Level 3 qualification (for band 3) or equivalent experience in a relevant subject, RDaSH are offering this post alongside the opportunity for you to complete the relevant apprenticeship.
As an apprentice within RDaSH you will be given the opportunity to gain experience, learn work-related skills and obtain all the tools needed to thrive.
An apprenticeship includes regular training with a college or other training organisation. Around 20% of your working hours will be spent training or studying. This leads to a nationally recognised qualification.

Main duties of the job
The successful candidate will be able contribute to the assessment, implementation and delivery of high-quality patient care within a professional team.
In addition, you will also demonstrate responsibility to patients, relatives and colleagues through contribution to the development and monitoring of agreed evidence-based practice and to the health and social care programmes.
- Undertake agreed activities as delegated by a registered nurse, with patients / service users under the
direct /In-direct supervision of the qualified staff. - Contributes to the assessment, implementation and delivery of individualised evidence based care.
- To maintain accurate records in the patient’s own home or electronically, record new problems as they
are identified and report all actions to nurse in charge/case manager. - The post holder is required to take personal responsibility to incorporate the 6 C vision of care into their
care delivery. - Provide feedback of current patient / service user status to appropriate members of the-professional
team.

Qualifications
Essential criteria
- English and Maths at level 2 or equivalent (GCSE 9-4 or A*- C or functional skills level 2)
- NVQ Level 2 in a care related subject or equivalent or willingness to undertake a relevant Level 2 Apprenticeship
- NVQ level 3 in related subject or willingness to undertake a relevant Level 3 Apprenticeship
Skills
Essential criteria
- Basic IT skills
- Ability to communicate effectively both face to face and in writing
- Good interpersonal skills
Desirable criteria
- Previous experience of supervision of staff
- Previous experience of working in Community setting
