This is a unique opportunity to shape a new role and make a real impact across South Yorkshire.
The South Yorkshire Mental Health, Learning Disabilities and Autism (MHLDA) Provider Collaborative is seeking an experienced professional to join its system-wide programme as an Acquired Brain Injury (ABI) Specialist Case Manager. The role has been created to address gaps in care for adults with ABI and co-existing mental health needs, improving outcomes and reducing out-of-area placements.
The Provider Collaborative brings together NHS organisations, commissioners, local authorities, independent providers and people with lived experience to develop specialist services and support people to receive care closer to home.
Hosted by Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH), this role will work across organisational boundaries to support the development of integrated, person-centred ABI pathways across South Yorkshire and beyond where placements are out of area.
You will manage a complex caseload, providing expert oversight, coordination and risk management while working with partners across the system. This is a rare opportunity to influence service transformation, shape future pathways and improve care for people with complex needs.

Main duties of the job
The post holder will lead the coordination and management of highly complex ABI cases across South Yorkshire, working collaboratively with people with acquired brain injury, families, health, social care, and partner agencies. You will undertake comprehensive assessments, develop and review care and risk management plans, and support safe, effective, and person-centred care delivery.
You will play a key role in reducing out-of-area placements by supporting repatriation planning and improving local care pathways, while also contributing to reduced length of stay and improved patient flow.
The role requires strong leadership and influencing skills, as you will provide expert advice to multidisciplinary teams, support service development, and drive quality improvement initiatives.
You will analyse complex clinical and service data, contribute to business cases, and help shape future models of care for people with ABI.
Working autonomously across organisational boundaries, you will manage competing priorities, respond to complex and high-risk situations, and ensure care delivery aligns with best practice and relevant legislation.

Qualifications
Essential criteria
- Professional qualification in a relevant health or social care discipline (e.g. Nursing, Occupational Therapy, Social Work, Psychology or equivalent)
- Current registration with the appropriate professional body (e.g. NMC, HCPC, Social Work England)
- Evidence of continued professional development (CPD) relevant to ABI, mental health, or neurorehabilitation
Desirable criteria
- Postgraduate qualification or equivalent experience in a relevant field (e.g. neurorehabilitation, mental health, leadership, or service improvement)
- Training in quality improvement methodologies (e.g. PDSA, Lean, Model for Improvement)
- Additional specialist training in risk assessment, behavioural management, or complex case management
Experience
Essential criteria
- Experience of working with individuals with complex needs, including acquired brain injury and/or mental health conditions
- Demonstrable experience of managing complex cases involving multi-agency working across health and social care systems
- Experience of undertaking comprehensive assessments, care planning, and risk management in complex clinical situations
Desirable criteria
- Experience of working within or across community, rehabilitation, or inpatient settings
- Experience of contributing to or leading service improvement, transformation, or pathway development
- Experience working at a system level (e.g. across ICS, provider collaboratives, or multi-organisational partnerships)
- Experience of managing or supporting repatriation from out-of-area placements
- Experience in influencing commissioning, service design, or business case development
Knowledge
Essential criteria
- Understanding of relevant legislation, including the Mental Health Act, Mental Capacity Act, and safeguarding frameworks
- Knowledge of risk assessment and risk management processes
Desirable criteria
- In-depth knowledge of acquired brain injury, including cognitive, behavioural, emotional, and social impacts
- Knowledge of neurorehabilitation principles and best practice in managing complex ABI presentations
- Knowledge of national policy, guidance, and best practice relating to ABI, mental health, and rehabilitation pathways
Skills / Abilities
Essential criteria
- Clinical assessment, formulation, and care planning skills
- Excellent communication skills, including the ability to convey complex and sensitive information to a wide range of audiences
- Ability to work autonomously and exercise professional judgement in complex and uncertain situations
- Strong organisational and prioritisation skills, managing competing demands across a complex caseload and system responsibilities
- Ability to build effective working relationships and influence across organisational and professional boundaries
- Ability to lead and contribute to service improvement and transformation initiatives
Desirable criteria
- Skills in data analysis, interpretation, and presentation to inform service planning and improvement
- Facilitation and negotiation skills, particularly in complex multi-agency environments
