Job overview
To assist and support the Senior Learning Disability and Autism Clinical Nurse Specialist (CNS) in working collaboratively across the multi-disciplinary team to increase access, reduce health inequalities and improve health outcomes for people with learning disabilities and autistic people who use services at the Royal Orthopaedic Hospital (ROH).
The Learning Disability Liaison Nurse is responsible for delivering high-quality, patient-centred care to individuals with learning disability and/ or autism diagnoses. The role focuses on assessing needs, planning and delivering care, and supporting patients to maintain independence, dignity, and wellbeing.
The post holder will undertake comprehensive assessments, develop and implement personalised care plans, and provide therapeutic and emotional face to face support to patients and their families. They will work collaboratively within a multidisciplinary team to ensure coordinated care, promote recovery, and to safeguard vulnerable individuals.
Main duties of the job
To work as part of the Vulnerabilities Team and support the Senior Learning Disability and Autism CNS
- To be a highly competent and knowledgeable practitioner within the learning disability speciality and provide specialist learning disability and autism knowledge to support patients to reduce health inequalities.
· To actively promote the Learning Disability Nurse role to service users, families and carers and other professionals.
· To work with children, young people and adults who are diagnosed with learning disabilities, profound and multiple learning disabilities and/ or autism, their parents, and carers to develop reasonable adjustment plans for outpatient services ensuring the provision of safe, high-quality services.
· To work as part of a MDT under the supervision of the Senior Learning Disability and Autism CNS to assist in the planning, implementation, and evaluation of comprehensive care plans pre-operatively, during admission and post-operatively ensuring a safe and effective discharge using a patient-centred approach.
· Be an advocate for patients within the Trust under the Learning Disability and Autism service.
· To escalate all complex cases to the Senior Learning Disability and Autism CNS for oversight.
· To provide face to face support and advice ensuring reasonable adjustments are in place to support the patient’s needs.
· To ensure the hospital passports are used and available across the Trust.

- Current entry on the NMC Register – Learning Disability (RNLD)
- Evidence of continuing professional development
Desirable criteria
- Knowledge of Safeguarding Vulnerable Adults and Child Protection and Mental Capacity Act and DoLs/ LPS
- Experience of delivering training
Experience
Essential criteria
- Experience as a band 5 or 6 nurse working with people with learning disability and/ or autistic people in NHS hospitals or community settings
- Understanding of health inequalities and health promotion
- Excellent written and verbal communication skills
- Working knowledge of clinical and safeguarding practices
- MDT team meeting experience
- An understanding of stress and carers responsibilities
- Experience of working with families and implementing individualised care plans
Desirable criteria
- Experience of managing complex casework
- Working with vulnerable adults and children
- Experience and evidence of implementing change
Knowledge
Essential criteria
- Demonstrate a knowledge of duties required for the post
- Demonstrate ability as expert role model in all aspects of work.
- Knowledge of legislation and guidance around learning disability and autism.
- To have awareness and understanding of the SEND process.
- Excellent communication/ interpersonal skills verbally and written, numeracy and literacy
- Confident in advocating for patients in an appropriate manner
- Adapt to wide range of people and situations

- Awareness of how gender, social, economic, race, cultural, linguistic, religious and sexual orientation issues may impact people’s lives
- Ability to use supervision effectively
