Job overview

Working as a senior member of an established Community Mental Health Team in the North Locality of Doncaster providing a high standard of safe, robust and evidence based clinical practice.

Working with complex organic and functional diagnosis within the community setting, liaising and integrating with both statutory and voluntary organisations in support of the specific client group.

Main duties of the job

To promote and maintain the highest attainable standard of nursing care in a community setting.

To support the band 6 nursing team with implementation of evidence based interventions from a plan of care

Perform re-assessments of clients to include risk and crisis plans

To maintain close working relationships with referring agencies.

To deputise for the Team Leader when required.

To carry a caseload of patients with both functional and organic issues as identified through the team systems process.

Qualifications

Essential criteria
  • Registered Mental Health Nurse with current NMC registration
  • Mental Health specific nursing qualification
Desirable criteria
  • Registered teaching /training qualification

Experience

Essential criteria
  • Demonstrable experience including working successfully with Older people suffering with Dementia and functional mental illness.
Desirable criteria
  • Evidence of inter-agency working, Lead professional organisation.

Knowledge

Essential criteria
  • Experience of cognitive testing, complex assessments, working with family and carers.
  • Management of cognitive enhancer treatments.
Desirable criteria
  • Experience in providing both clinical and management supervision

Skills

Essential criteria
  • Demonstrate advanced comprehensive communication skills
  • Clear/concise command of English written/oral
  • Ability to formulate plans for service strategy.
  • . Co-ordination of multi-professional team activities.
  • Proficient with IT skills.
  • Displays a positive attitude, motivated.
Desirable criteria
  • Specialised skills e.g. appraising, interviewing, counselling skills.
  • Report writing, knowledge of Policies and Procedures for the Trust
