Job overview
The post holder will work as a member of a multidisciplinary within the Adult Community Mental Health Treatment Team, providing a service to those experiencing mental health symptoms of a serious and enduring nature.
To be responsible as Key Worker for the assessment and co-ordination of care needs, planning and implementation of evidence-based care to a defined group of service users and their carers.
To provide professional advice and support to other agencies and members of the multidisciplinary team.
To act as a role model and resource for junior/less experienced staff.
Main duties of the job
· To be responsible for the assessment, planning implementation and evaluation of the evidence based care required, including health promotion for a named service user/group.
· Responsible for the appropriate clinical risk assessment of service users contacting the crisis service as appropriate.
· Providing an effective advice, guidance and decision making process culminating in the co-ordination of any reviewed plan of care.
· Responsible for the admission, case conference/reviews and discharge in collaboration with members of the multi-disciplinary team.

Qualifications
Essential criteria
- Degree, Diploma or equivalent in R.M.N, and significant pre-qualification experience plus additional training/short courses
Desirable criteria
- Continuing post qualification Mental health training
- Experience of working with People with mental health problems
Experience
Essential criteria
- Appropriate experience post qualification in the field of mental health
- Crisis working Management of complex needs
Desirable criteria
- Work in multi-disciplinary Setting
- Experience of working with Older people with mental health problems
- Local Government experience Mental Health experience Care Programme Approach Care co-ordination.
Knowledge
Essential criteria
- Understanding of Equal Opportunities’ Policy.
- Knowledge and understanding of current mental health issues.
- Knowledge of the National Health Service and related organisations.
- Knowledge of the Mental Health Act and Mental Capacity Act and Care Act including other relevant legislation
Desirable criteria
- Knowledge of Dialog plus
- Experience in delivering psycho-social interventions
- Experience in mental health work
Skills and abilities
Essential criteria
- Ability to manage a large case load and co-ordinate care as a key worker
- Assessment and organisational skills. Ability to prioritise work. Range of intervention skills. Problem solving skills.
- Evidence of co-ordinating /planning interagency approaches to mental health care.
- Ability to work as part of a team and also without direct supervision.
- Excellent communication skills in individual and group settings High standards of literacy and IT skills.
