The Registered Nurse (RN) will participate in the inter-disciplinary care of a caseload of patients 
whose needs are being met within the intermediate care pathways   This will include shift 
coordination, team leadership, case management and direct care delivery in a variety of settings. 
The RN will act as a named nurse in the assessment, planning implementation and evaluation of 
nursing care for an identified group of named patients in keeping with the RDaSH definition of 
Named Nurse.  This will be achieved by following evidence based, individualised plans of care. 

Main duties of the job

To be responsible for   the delivery of person centred plan of care e.g. assisting with 
activities of daily living, MDT goal setting,  admission & discharge planning, administration 
of medication etc on a daily basis. 
To be responsible for the leadership of the care team on a shift basis to ensure the safe 
and effective care delivery of a group of patients through delegation and shift / team 
coordination. 
To be responsible for the assessment, planning, implementation and evaluation of 
individualised evidence based care with minimal ongoing supervision of the ward sister. 
To work as part of the MDT in ensuring the safety of patients to include specific risk 
assessment and risk management planning

Qualifications

Essential criteria
  • Current and active registration with NMC
  • Registered General Nurse
Desirable criteria
  • Evidence of post-qualification studies
  • Mentorship qualification
  • Non-Medical Prescribing

Experience

Essential criteria
  • Experience of working in a professional nursing role with patients with physical health needs
  • Knowledge and experience of working in a multidisciplinary setting
  • NHS experience
  • Experience of working with patients who hav long term coniditions
Desirable criteria
  • Experience of working in an intermediate care/rehabilitation setting with relevant transferrable skills
  • Experience of working within a hospital setting

Knowledge

Essential criteria
  • Valid and current evidence of education and training to achieve registration as Nurse with NMC - current students due to complete will be considered
  • Evidence of learning from reflection and self-directed study
  • Good awareness of NHS policy, national strategies and legislation including Mental Capacity Act and evidence based practice in Rehabilitation/intermediate care
  • Ability to work single handily without direct supervision and to organise own workload, and delegate appropriately
  • Understanding of team dynamics and an awareness of how to be an effective team member
  • Effective written and oral communication skills, including the processing of complex and sensitive information
  • Basic word processing and internet skills
  • Basic knowledge of the principles of Clinical Governance
  • Good personal organisational skills, including effective planning and organisation of self and patient care

  • Health and Safety, and Security awareness
  • Good communicator - customer service skills
  • Ability to keep accurate and legible notes

  • Extended scope of skills/ training: venepuncture, cannulation, IV drug administration, Male/Suprapubic Catheterisation, Non-medical prescribing, mentorship

  • Evidence of speciality specific education/skills/competencies: Neurology or Rehabilitation
  • Clinical assessment skills
