Job overview
To support the integrated 0-19 Healthy Child Service (HCS) in providing an innovative and high quality service to improve health outcomes for children, young people and their families
To support the assessment and review of clinical needs
To work without direct supervision to support clients as part of the universal and Targeted aspects of the service provision
To liaise with colleagues within the clinical team to assist in the coordination and development of the service.
To develop specialist interest areas in order to enrich the knowledge and skills set within the Norfolk Children & Young People's Health Service.

Main duties of the job
To assist the School Nurses in the management and delivery of care to clients. Duties may include:
Contacts with young people will be delivered in a variety of ways such as families homes, and venue based such as schools at a variety of locations across the collaborative localities
Building on existing assessment of families’ needs and deliver evidenced based health advice without direct supervision.
Receiving referrals, triaging and using appropriate sign posting.
Working jointly with School Nurse for Specialist families. This may include:
- Communicating complex information with other agencies such as social care staff regarding work HCS are delivering to the young person.
- Support to clients with more complex needs.
- Completing Duty – reviewing waiting lists, triaging tasks, booking in contacts and running reports within system one.
- Working jointly with outside agencies who may also be supporting a young person.

Detailed job description and main responsibilities
To support the integrated 0-19 Healthy Child Service locality team in providing an innovative and high-quality service in an environment that enhances client care and promotes child and family wellbeing.
To support the assessment and review of clinical needs in the 5-19 Team.
To work both as a team and without direct supervision to support clients as part of the universal and targeted aspects of the service provision.
To support the assessment and review of clinical needs.
The post will be working over collaborative localities but will need to have flexibility to support other localities as needed to ensure that service delivery is equitable.
To liaise with colleagues within the clinical team to assist in the co-ordination and development of the service.
To develop specialist interest areas in order to enrich the knowledge and skills set within the Norfolk Children & Young People’s Health Service.
Maintain continued development and learning to ensure that practice remains current, evidenced and of high quality for the children, young people, and families we support.
To assist in the prioritisation of referrals and booking appointments.
To engage in team and locality meetings.
To offer an inclusive service that is accessible to all recognising individuals’ equality, diversity and rights.
Assist in the delivery and planning of programmes/activities of care that may be with individuals or to groups such as parents/carers.
To provide targeted packages of care under the supervision of a registered case holder. Use goal setting to support the service user to identify their own needs and empower them to make sustainable healthy choices.
To write clinical records in a concise, accurate and timely manner in line with East Of England Community Health and Care record keeping policy.
To promote the use of digital resources on Just One Norfolk, to enhance self-care skills for children, young people and their families.
To confidently utilise virtual platforms in assessments, interventions, training, and networking to support effective practice and partnership working.
To support Duty in localities on a rotational basis.
Within this role you will need to travel as an independent practitioner on a daily basis, across Norfolk which involves both rural and urban travel. The nature of our work does result in travel arrangements changing at short notice.
Due to the large geography our service covers and the limitations of sufficient public transport to undertake this role you must hold a full and current driving licence and have access to your own transport. This is subject to the provisions of the Equality Act 2010, and therefore if the postholder requires a reasonable adjustment due to a disability then this will be considered.

Qualifications
Essential criteria
- NVQ level 3 in health, social care, education or an equivalent.
Desirable criteria
- Foundation degree in relevant area.
Experience
Essential criteria
- Recent experience of working with children and young people and families.
- Building professional relationships with parents/carers
- Demonstrates the ability to safeguarding children.
Desirable criteria
- Experience of working with families in the community
- Planning and facilitating groups to deliver health promotion messages
Skills
Essential criteria
- Good communication skills and have the ability to change communication style depending on audience.
- Ability to analyse information and use reasoning skills to assess clients needs.
- Robust IT skills and clear record keeping.
- Understanding of the public health agenda and national and local policies relating to Children and Young People’s health and wellbeing, including Working Together to Safeguard Children.
- Understanding key public health resources to support the children and families of Norfolk.
Desirable criteria
- Ability to apply and use assessment tools to support a childs developmental assessment and identify need
- Knowledge of Systm1
- Experience in or knowledge of child development
