Older Peoples Team Manager

Job overview

This is a fantastic opportunity to take up an exciting and innovative post to support the needs of the local population living with a wide range of mental health difficulties. The post holder will provide operational management to Rotherham care group's Older People's Community Mental Health services. The Team manager will work in a collaborative and multi-agency way and develop multiagency and multidisciplinary operational frameworks and structures.

This role provides expert operational and clinical leadership and knowledge within the teams, for staff, patients and carers. The role will work with with a variety of roles including  Nurse Consultants, Nurses Occupational Therapists, Healthcare support workers, Doctors and other external and internal stakeholders. 

Main duties of the job

To be responsible for the operational management within the Older People's Community services.  This primarily would include management of the Care home liaison Team, Physiotherapy, Young Onset Dementia service and Intermediate Care (Mental Health). It could also include operational management of the other teams within the Older Adults community portfolio. The post holder would also be responsible in supporting Transformation and service development.

Provide strong leadership for all staff within the Older People's services by

  • Promoting and maintaining a quality environment
  • Ensuring agreed standards of care are achieved
  •  Improve patient experience, and 
     achieve all performance targets relating to the teams
  • Effective communication, risk management and considered risk taking.

To provide expert clinical and management advice and guidance for staff and external stakeholders.  

To focus care on Quality and Clinical Assurance as outlined in the key roles and responsibilities for clinical
services.

To be responsible for coordinating staff training to achieve quality and evidence based interventions based upon a skills/training analysis. To arrange and coordinate training for staff on various relevant clinical skills.

To undertake investigations in line with the relevant organisational policy (e.g. complaints, Incidents, Serious Incidents, Structured Judgement Reviews, HR investigations).

Experience

Essential criteria
  • Capability to lead & manage improvements in patient care across care pathways
  • Confidence to discuss patient management / express difference of clinical opinion with other members of the multiprofessional team.
  • Willingness to sensitively challenge & effectively address ineffective/weak/poor practice/systems where it/they occur.
  • Being a strong role model & leader, inspiring others to deliver safe, effective and personal care every time.
  • Experience of engaging in service/quality improvement/evaluation work, research and or clinical audit.
Desirable criteria
  • Experience of working in a broad / diverse range of care settings / services / organisations.

Knowledge

Essential criteria
  • Experience of collaborative working across agencies and professional disciplines.
  • Experience of providing supervision to staff.
  • Experience in managing Complex Cases and Risk.
  • Knowledge and understanding of older people with mental health problems and/or dementia
  • Experience of working in mental health settings including older adult services
  • Knowledge of essential physical health care for people experiencing severe mental illness. assessment & management.
  • Professional Code(s) of Conduct & requirements.

Qualifications

Essential criteria
  • Diploma / Degree in relevant qualification
  • Registered Mental Health Nurse, Occupational Therapist or Social Worker
  • Evidence of Continuous Professional Development.
  • Evidence of Statutory & Mandatory Training
Desirable criteria
  • A recognised qualification in Clinical Supervision.
  • Leadership/management qualification.
  • Evidence of ability / capability to study at Post Graduate level

Skills

Essential criteria
  • Passionate and committed to staff well-being and supporting people who use our services.
  • Experience of providing a range of evidence-based interventions to people with a variety of mental health problems and serious mental illness.
  • Demonstrates the ability to develop & educate other members of the multi professional team to improve patient care & outcomes, leading by example.
  • Strong liaison and negotiation skills.
  • Competent in the use of IT / computer work-based information & care record systems.
Desirable criteria
  • Skills acquired in a breadth / diverse range of care settings / services organisations.
