The Mental Health Division of Nottinghamshire Healthcare Foundation Trust comprises of three directorates; Adult Mental Health, Mental Health Services for Older People and Specialist Services. Main duties of the job

Support the Dementia Outreach Service with the implementation of patient care plans, support and advice for care homes within the Broxtowe area.

Monitor and record individuals' mental health status and report any significant changes.

Assist individuals in developing and maintaining coping strategies to manage their symptoms.

Seeing patients who are permanent residents in Care Homes within the Broxtowe  area. 

Carrying out duty which involves making and receiving phone calls from care home staff  offering support and guidance, reviewing their electronic records, and making entries.

Attending meetings face to face and on MS Teams.

Be a member of a multidisciplinary Team.

Detailed job description and main responsibilities

This is an exciting opportunity, to be part of a specialist Outreach service for clients with dementia in residential/nursing care providing specialist nursing/healthcare advice, treatment and teaching/education as appropriate under the guidance of professionally registered team
members. To contribute to assessment and interventions carried out by the dementia outreach. to engage with the Enhanced Health in Care Homes Service (EHCH) and with GPs in the local area.

Part of a multi-disciplinary Outreach service which works with both Independent Sector and Statutory Sector providers of Residential Care to Older People with dementia. To increase the capacity, confidence, competence and willingness of those organisations to provide the right level of care to this vulnerable client group and by doing so to increase standards of care, improve quality of life, increase independence and choice.  You will be supported through training and supervision.

As this role is community based covering the Broxtowe area, a full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Please note that this post does not meet the pay level required for a Skilled worker visa. Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. 

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

Qualifications

Essential criteria
  • NVQ level 3 Health and Social Care Certificate or equivalent experience

Knowledge

Essential criteria
  • To have a basic understanding of mental health conditions
  • To maintain an understanding and caring approach to patients
  • To have an understanding of dementia.

Skills

Essential criteria
  • Ability to work as part of the team
  • Good written/oral communication skills
  • Diplomatic, caring and sympathetic
  • Develop observations skills with the ability to report and record physical/mental health changes ect
  • IT skills
  • Role modelling dementia care approaches

Experience

Essential criteria
  • Experience of working in a Healthcare setting with older people who experience mental health issues
  • Experience of working in a Healthcare setting with people living with dementia.
