The role of the post holder will be to work in the Trust acute inpatient Tobacco Treatment Service. The post-holder will provide day-to-day support in this direct patient-facing role to provide behavioural support and treatment in relation to tobacco smoking.  In addition, the role will focus on the training and education of colleagues around Tobacco use and the inpatient treatment model: ‘Ask, Advise, Act’.  The role requires the ability to work in any given department throughout ELHT, including emergency care pathways in providing ‘safe, personal and effective’ care to all identified Tobacco users.

 

Successful candidates will organise their own workload responding to the needs of the service and be able to work unsupervised within a busy team. This will include taking referrals, providing psycho-social support and making onwards referrals for continued outpatient support. There will be a strong emphasis on the provision of training and advice to clinicians and in raising awareness around the harms of Tobacco use and opportunities available to help people lead healthier smoke free lives.

1.    Take referrals and work with patients to identify the most appropriate Tobacco dependency treatment interventions.

2.    Proactively target potential referrals within the Acute inpatient setting through the agreed Trust screening protocol for Tobacco use.

3.    Conduct assessments, identifying potential risks, advising on appropriate Tobacco dependency treatments alongside behavioural support.

4.    Provide support to individuals to make informed choices about their health and wellbeing and support them in initiating and sustaining appropriate behavioural changes to improve their health.

5.    Support those who use Tobacco through onward referral to access community Tobacco treatment and pharmacy services and pharmacy smoking cessation pathways ensure smooth transition and follow up post discharge.

6.    Deliver training to colleagues on best evidence around stop smoking interventions.

 

7.    Support the delivery of the service key performance and reporting indicators for tobacco dependency, recording and reporting activities and results contributing to achieving all performance indicators, essential standards and quality outcomes.

8.    Engage with partnership agencies external to the Trust to reduce the risk of relapse and ensure continuity of care.

9.    Accurately collate and input data into the database for the team.

10.  Support clinical and non-clinical areas to develop a smokefree culture and to adhere to the Trust wide Smokefree policy.

  1. Substantial knowledge around the use of Tobacco, other substance use associated harms, health complexities and treatments.
  1. Ability to practice effective motivational interviewing skills.
  1. Demonstrate experience in delivering evidence based psychosocial interventions to support health behaviour change.
  1. Attend training relevant to the post to enhance competence.
  1. Ability to deliver training to a range of professionals.
  1. Hold excellent IT skills including database input.
  1. Self-awareness around limitations of skills and competencies and ability to work within them.
  1. Knowledge of Tobacco related harms and treatment options.
  1. Experience of working in health promotion/healthcare setting
  1. Ability to use carbon monoxide measurement including interpretation of the results and record data as appropriate.
  1. Where appropriate, give advice on the provision of pharmacotherapy such as Nicotine Replacement Therapy (NRT), acting as a resource in line with locally agreed protocols. 
  1. Understanding of health inequalities and the wider determinants of health.
