Are you a Band 6 Occupational Therapist or a Band 5 Occupational Therapist with 
some experience, looking in to developing/ progressing into a Band 6 OT role? If 
the answer is “yes”, we would love to hear from you!
Barnet Planned Care provides more integrated care and rehabilitation in the 
community. As a Multidisciplinary team of Occupational Therapists, 
Physiotherapists, Community Nurses, Rehab Support Workers, Technical 
Instructors, Case Managers and Pathway Support Workers; we provide 
rehabilitation to adults over 18 years of age, helping them to regain independence 
following an illness or injury including falls and stroke. We also aim to prevent 
any avoidable attendance to A&E, admissions to hospital, manage long term 
conditions and promote health and wellbeing.
Barnet Planned Care provides integrated falls and intermediate care services 
which includes clinic-based specialist falls assessments, functional assessments, 
environmental assessments, and rehabilitation at home for patients, their families, 
and carers to regain independence in their activities of daily living for up to 6 
weeks.

Main duties of the job

It is essential that you have current HCPC registration as an Occupational 
Therapist. If you are applying for a Band 5 developmental OT, you should have some post qualification experience of working with people with physical disability and for Band 6 applicants you should have significant experience in working with people with physical disability, experience of working in multi-professional teams and across a range of different agencies in the community; and should be able to  demonstrate clear understanding of specialist assessments and treatment skills, person cantered goal setting, and falls management. 
As a B6 or a Band 5 Occupational Therapist you will also be responsible for 
providing supervision, support and education to other junior therapists and 
rehabilitation support workers. You will have the opportunity to provide clinical practice education opportunities to students on placements. There would be the chance to be involved in service development opportunities and audits.

Detailed job description and main responsibilities

  • To undertake specialised uni-professional and multi-professional assessments. To develop and implement treatment plans for clients referred to the Barnet Intermediate Care Service including Falls and Early Stoke Discharge Service. These clients may present with a variety of complex multi-pathologies within specialist areas, including neurological conditions.
  • To participate in the Single Assessment Process when appropriate, performing overview assessments and complex specialist assessments as appropriate. To communicate relevant medical information, assessment details, advice and recommendations across agencies and with clients in compliance with trust guidelines for sharing of information and informed consent. These agencies include General Practitioners, Social Services, healthcare professionals, voluntary agencies, and residential homes.
  • To be responsible for gaining valid, informed consent for assessment, treatment interventions and sharing of information in compliance with legal and professional guidelines. To be able to interpret and follow these guidelines in circumstances where clients do not have capacity to provide informed consent.
  • To have working knowledge of clinical guidelines; specific to the area you are working in and, with the guidance of your supervisor, to translate these guidelines into clinical practice.

Essential criteria

  • • 1.3 Evidence of up-to-date CPD portfolio
  • • 1.4 Involvement in Appraisal Scheme
  • • 1.5 Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology, Older People Rehabilitation
Desirable criteria
  • • 1.6 Post graduate degree in relevant area
