The Physiotherapy Department is a dynamic, friendly and cohesive department and we are looking to recruit a motivated and ambitious individual to work within our Hospital Outreach Support Team (HOST).

HOST is a highly regarded and experienced team supporting discharges and helping prevent readmission of complex patients from all specialities and demographics. By reviewing patients in their own homes, HOST provide short term patient centred rehabilitation, adaptive aids and advice to help patients reach their functional goals. In addition, HOST support patients with management of orthopaedic devices including cervical collars, upper and lower limb braces. 

We are looking to recruit an experienced band 5 or current band 6 physiotherapist for a full time position. You will demonstrate excellent multidisciplinary team working, adaptable communication for complex patients and be innovative in their approach to support further development of this busy, dynamic department. 

Benefits of the role include the opportunity to experience a wide range of complex patients with varied pathologies and functional abilities; close collaborative working with occupational therapy; supporting the patient pathway between the acute and community settings and experience with a wide variety of orthopaedic devices and adaptive aids. A rotation with inpatient services will be considered.
