Job overview
An exciting opportunity has arisen for an autonomous and highly motivated nurse to join the Memory Assessment Service. This service covers the East Noth East Leeds area. The role involves the assessment, diagnosis and post diagnostic support of people with or suspected Dementia. You will independently assess people and work with their loved ones, primarily at home, to make a comprehensive judgement on the possibility of cognitive impairment. This assessment will then be fed back to the wider collaborative of MDT staff, enabling timely diagnosis and post diagnostic support. This position does not hold a caseload. You will be encouraged to undertake professional development and be supported in participating in the PDP process, clinical and management supervision and as an active member of the team you will also be involved in service improvement initiatives within the wider Care Group.
Being a car driver is essential to this post.
Main duties of the job
We are currently looking for Band 5 Nurse to join the Leeds Memory Services. This is a city wide service with a focus on assessment and diagnosis of dementia serving to meet the needs of the people of Leeds who are experiencing memory problems however, this position is for the East Leeds locality. This service provides the diagnostic element to working with people with dementia and does not include any ongoing care coordination. If ongoing support is required the post holder will be responsible for all necessary referrals, there is no caseload element to the work. You must be able to demonstrate a passion for working with people with Dementia, be committed and motivated to working with older people and take an active role in service development. The post holder must be able to act as a role model for others, and demonstrate a high level of clinical judgement, decision-making and communication skills. They must have a solid understanding of autonomous working, effective risk assessment and be able to manage competing demands to ensure timely assessment and written documentation. We welcome applications from people who have a strong commitment to working with older people using evidenced based practice and are prepared to develop their professional skills and knowledge. We also have a strong commitment in helping staff build on their experience and skills through reflection in appraisal objectives, management and clinical supervision and personal training and development.
