Job overview

Are you a community nurse experienced in assessing patients in their own homes? Do you have knowledge of palliative and end-of-life care, with a real passion for improving quality of life? We want to hear from you.

We’re looking to grow a small team of Band 5 Palliative Care Support Nurses across Cornwall. In this role, you’ll work alongside the Specialist Palliative Care team, taking responsibility for assessment, care planning, implementation, and evaluation under the guidance of a Specialist Palliative Care Nurse.

Your role will involve supporting patients, carers, and families in their home environment, helping them maintain independence and achieve the best possible quality of life.

You’ll work via our staff bank, to cover temporary shortages in SPC caseloads due to sickness or staff changes. This position is a great opportunity to develop specialist skills and gain insight into the Specialist Palliative Care Nurse role.

Main duties of the job

Responsible for the assessment, planning, implementation and evaluation of care, under the remote management of a Specialist Palliative Care Nurse, to patients, carers and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life.

To work in partnership with patients and carers to offer support and advice and promote informed choice.

Sponsorship is not available for this position as it does not meet the UKVI eligibility criteria for a skilled worker visa.  Any applications from individuals who require sponsorship to work in the UK will therefore be declined.

Working for our organisation

We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly.  We deliver community and hospital-based care to improve people’s physical and mental health.  We also provide specialist support to people with dementia or a learning disability.

We are a people organisation and people matter to us.  As part of the team, you’ll help support the health and wellbeing of the people who live and visit this beautiful part of the UK.

Over 4,000 people make up the Trust.  This includes doctors, nurses, therapists, plus admin and support staff.

We work in people’s homes, in community clinics and bases.  Some staff work from one of our 13 community hospitals.  Our aspiration is to have great people, provide great care, be a great place to work and a great partner.

Approximately 568,000 people live here.  A third of people who live in Cornwall are supported by acute hospital services in Devon.  As a result, we also work closely with our partners in Devon.  In the summer, and during other holidays lots of people choose to visit the area.  This increase the numbers of people who use out services.
