Central London Community Healthcare – Band 3 Bank Healthcare Assistant
The Clinical Support Worker (CSW) will provide a combination of general and specific care to a group of patients under the guidance and supervision of a registered nurse or allied health professional.
The post holder is expected to develop proficiency and competence in their area of practice through competency assessments specific to their clinical area. The CSW will promote optimum independence and deliver individualised, patient-centred care to a delegated caseload.
Additionally, the post holder will support the Community Nursing/Rehabilitation Team in achieving key performance indicators and maintaining the quality of service delivery.
Main duties of the job
- To contribute to the on-going assessment of a patient by completing the appropriate paperwork for allocated patients, recording and reporting all clinical and behavioural information to the nurse/therapist for action and planning purposes.
- To undertake, and report back on delegated clinical activities that have previously been signed competent. Eg: vital signs, routine urinalysis, glucose monitoring, gait/mobility training, falls prevention awareness groups, increasing independence through activities of daily living
- To enable patients/carers to make most effective use of resources within the service by ensuring they receive up-to-date information sheets, responding to any questions constructively
- To pass onto/seek assistance from other colleagues, promptly, any questions that you are unable to resolve.
- To liaise and refer patients to the MDT team as appropriate in accordance with local procedures
- To assist with errands and general duties appropriate to the clinical area as required.
- To prescribe appropriate equipment and educate carers and patients in the safe use of this equipment, after being deemed competent to do so.
- To use clinical judgement and skills to report any untoward changes in carers or client’s health status immediately to the relevant team member, GP, District Nurse or emergency services if appropriate.

Education/Qualification
Essential criteria
- Higher Professional Diploma in Health and Wellbeing (NVQ/QCF – Level 3)
- A recognised equivalent course
Desirable criteria
- GCSE grade A-C or equivalent in English and Maths ECDL certificate
- Registration with the national regulatory body (when it becomes available).
Experience
Essential criteria
- Experience of working in a health or social care setting
- Experience of working as part of a team
- Experience with the use of Microsoft Office and a variety of databases.
Desirable criteria
- Experience with the NHS patient information database (Rio, CIS)
Skills & Knowledge
Essential criteria
- Knowledge of NHS Demonstrate an acceptable level of numeracy and literacy
- Knowledge of multi-agency services available and processes for referral
- An awareness of risk management.
- Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
Desirable criteria
- Knowledge of local health needs strategies.
