An opportunity has arisen to join our Community Nursing Team as a Assistant Practitioner working as an integral part of a Community Nursing service within the Newbury locality.

Main duties of the job
-
Deliver clinical and therapeutic care to patients within agreed trust policies/ protocols, as documented in the band 4 competency assessment tool.

Contribute to the holistic assessment and management of patients care needs through initial data collection, monitoring of patients progress, feedback discussion reports and in care reviews with the Community Nursing Team

Develop, implement and evaluate nursing plans in response to identified needs of the individual, carers and community in partnership with clients

To provide care according to the available evidence of best practice.

To identify and plan preventative measures and anticipatory care needs in discussion with registered nursing staff.

Accurately use contemporary nutritional assessment tools and record fluid intake to identify signs of dehydration or fluid retention and escalate as necessary

To be involved in the management of End of Life care and work in partnership with health, social and voluntary agencies to ensure patients are treated with dignity and respect, and to support their families and carers.

Education/Qualifications/Training
Essential criteria
- Registered Nursing Associate on the NMC register
- Evidence of the Nursing Associate qualification
- Evidence of Education to a minimum level 5 foundation degree specific to nursing associate practice
- GCSE level 4-9(Grade A-C) in Maths and English or equivalent
Desirable criteria
- Care certificate assessor
Continuous Professional Development
Essential criteria
- Previous experience of working within a health / care setting
- Evidence of up to date relevant CPD
- Understands requirements for NMC revalidation and shows evidence of on-going professional development through maintenance of NMC revalidation
- Ability to engage and complete Berkshire Healthcare preceptorship programme
- Committed to the advancement of nursing
