We have an exciting opportunity for a pharmacist with drive and enthusiasm to join our Clinical Pharmacy team as the Lead Specialist Pharmacist (8a) for Homecare and Day case specialities.
The post holder will work closely with the Principal Pharmacist for Business and Transformation and will also be embedded in the multidisciplinary teams which are responsible for both Homecare and Day-case specialities. This individual will be responsible for providing comprehensive clinical pharmacy services as well as being at the forefront of service development.
We are looking for an experienced pharmacist with extensive clinical pharmacy knowledge and excellent communication skills. You will need to have the ability to work within a multidisciplinary team, driving change in practice and have a keen interest to actively participate in the training and development of others.
The department is supportive of further education, including leadership and prescribing qualifications if not already held, and you should be keen to continue your professional development.

To provide strategic and operational leadership for the clinical pharmacy service within designated specialist areas. The post-holder will be directly responsible for the oversight, delivery and continued development of services in:
• Homecare, with a primary focus on clinical areas currently without dedicated specialist pharmacist support, including endocrinology, immunology, lipid clinic, Clinical investigation unit (CIU) , dermatology and neurology
• MDU
• Day-case specialist services, as determined by departmental priorities
• Patient Group Directions (PGDs) and Patient Specific Directions (PSDs)
• Radiology and Endoscopy pharmacy services
The post-holder will ensure that patients across these specialties receive safe, efficient and patient-centred pharmaceutical care, underpinned by robust medicines management and strong clinical governance.
They will establish and maintain a clear framework to support the safe, clinically effective and economically responsible use of medicines, ensuring all services comply with organisational, regulatory and professional standards.
Works with significant autonomy, interpreting broad organisational policies and making decisions on service development and governance without direct supervision.
They will be accountable to the Chief Pharmacist for PGDs and PSDs, including their development, governance and review.
Deputising for Pharmacy Business and Transformation Manager.

To lead and provide high-quality clinical pharmacy services for Homecare and day-case specialties, ensuring the safe, effective and evidence-based supply, prescribing and monitoring of medicines for patients within these settings.
The post-holder will participate in Consultant-led ward rounds when required and, where appropriate, practise as an independent non-medical prescriber to optimise patient care. They will maintain strong multidisciplinary relationships, ensuring clear, timely and effective communication across the wider MDT.
They will identify opportunities for clinical development and service enhancement, presenting proposals to the DCSS Triumvirate management team and supporting the planning and implementation of changes that meet patient needs and maintain sustainable service income.
The post-holder will implement local and national Homecare guidance in partnership with clinical teams, ensuring that policies, pathways and procedures are current, accessible and effectively communicated to the wider workforce.
They will act as the pharmacy department’s subject matter expert for Radiology and Endoscopy, supporting the wider clinical workforce in these areas.
As governance lead for Radiology and Endoscopy, the post-holder will represent pharmacy at divisional governance meetings, take forward actions arising, manage risks, and ensure full compliance with medicines legislation and national standards.
They will communicate complex and highly specialist pharmaceutical information to healthcare professionals, patients and carers, adapting information to individual needs to support safe treatment, informed decision-making and medication adherence
The post-holder will lead on the creation and delivery of the clinical pharmacy audit programme, ensuring audit outcomes are translated into service improvements and best practice.
They will also supervise, mentor and train pharmacists, pharmacy technicians on rotation, and other healthcare professionals working within the specialist areas, ensuring high professional standards and continuous development within the pharmacy workforce.

Qualifications

- Masters degree in pharmacy or equivalent and 1 year pre-registration training.
- Registered Pharmacist with the General Pharmaceutical Council (Great Britain) as attained following successful completion of 1 year competency assessment and examination.
- Post-graduate Diploma in Clinical Pharmacy or equivalent.

- Registered independent non-medical prescriber.
Knowledge

- Professional knowledge acquired through vocational master's degree in pharmacy (4 years) or equivalent.
- Advanced level core therapeutic knowledge and clinical skills as gained through Post-graduate clinical diploma (or equivalent)
- Demonstrates high level of clinical knowledge, clinical reasoning and judgement; manages difficult and ambiguous problems.
- Understanding of background to and aims of national and local healthcare policy/guidance and appreciate the implications of this on engagement and priorities.
- Specialist clinical knowledge and skills relevant to the area of practice as developed through experience and CPD including conferences, courses etc.
- Demonstrable experience as a specialist practitioner in hospital clinical pharmacy.
- Experience as an advanced level clinical pharmacist practitioner, able to demonstrate a high level of competency e.g. equivalent to achieving Excellence in the majority of competencies in all clusters within the Advanced Level Competency Framework, except Management and R&D, which should be at Foundation level (as a minimum).
- Variety of clinical practice including medicine and surgery as a specialist.
- Evidence of working effectively within multidisciplinary groups.
- Previous experience of delivering training and education at post-graduate level.
- Evidence of activity in clinical audit or drug use review to improve practice.
- Current developments in pharmacy.
- Changes occurring in the NHS.
