Community Mental Health Nurse

Our Vision is to provide the best mental health, learning disability, autism, community, and physical health services for the populations we serve.

As an integrated Trust, Lancashire and South Cumbria NHS Foundation Trust (LSCFT) delivers a comprehensive range of services, including:

  • Primary and secondary mental health care for children and adults, including specialist inpatient child and adolescent mental health provision, perinatal mental health, and forensic services with low and medium secure care.
  • Specialist mental health inpatient care for individuals with learning disabilities, alongside specialist community support for children and adults with learning disabilities and autism, including intensive support.
  • Extensive community physical health and well-being services for children and adults, covering prevention, treatment, rehabilitation, and long-term condition management.

The Trust was first established in 2002 and employs approximately 7,500 staff who provide care from more than 400 sites. The organisation offers opportunities for medics, mental health, general nurses, children’s nurses, allied health professionals, psychology, administration and clerical staff, apprentices and volunteers, as well as those specialising in learning disabilities and community physical health services.

An interesting and varied part time vacancy (26 hours) has become available for an innovative and dynamic Band 6 Mental Health Nurse Practitioner to work across for the Older Adult CMHT in Lancaster and Morecambe.

The successful candidate will have sound clinical and leadership skills, be enthusiastic about the role and its development and be driven to provide excellent mental health services to the local older adult population

Prif ddyletswyddau'r swydd

Ideally applicants should have experience of working with older adults with mental health needs and the ability to lead on assessment and provide treatment plans. They should also have excellent communication skills.

The role will involve working within a MDT to support older adult service users on the dementia diagnostic pathway and in the community to remain at home and prevent hospital admission.  Please see the job description for further details about the role and expectations.

  • On-going registration with the NMC.
  • Specialist qualification or equivalent level in skills and development
  • Evidence of commitment to continuing professional development
  • Mentorship qualification

Knowledge

  • Understanding of Health and Safety policies and procedures
  • Knowledge and practice of Mental Health Act 1983 and the Mental Capacity Act 2005
  • Demonstrate understanding of how research has influenced practice

Experience
