Community Psychiatric Nurse (Developmental Role)

This is a fantastic opportunity to join a well-established OPMH service which includes a Community Mental Health Team, Memory Clinic, Home Treatment Team and Social Care service as a band 5 developmental role.
Bracknell is one of the Frimley Health Primary Care Network sites which is supporting the transformation of services in this area. This makes it an exciting time to join Bracknell as the service is collaborating with new teams who aim to work together to deliver services in line with the NHS Long Term Plan to better meet our client’s needs.
You will be participating in the provision of on-going assessments, care planning, treatment, and support for people with serious and enduring mental health problems and people experiencing a mental health crisis to promote their mental and physical health, independence, and recovery.
Location: The post holder will be a member of the Bracknell Community Mental Health Team for Older People and as such will be based mainly at Church Hill House in Bracknell, although flexible working opportunities can be available. The post holder is eligible for High Cost Living Allowance.
Shifts/Hours: You will also be part of the Home Treatment Team (HTT). Current shifts are 09.00 – 17.00 or 11.00 – 19.00 and 09.00 -15.00 on Saturday, Sunday and Bank Holidays.

- To be accountable and responsible for a defined case load. To work independently with those service users and their carers to effectively plan, monitor and evaluate interventions using evidence-based practice which is relevant to individual needs. Whilst discharging professional responsibilities under the Mental Health Act (1983) Mental Capacity Act (2005) and the NMC Processional Code of Practice and Guidelines and Standard 5 of the National Service Framework.
- Ensure that care plans and risk assessments are completed for service users. Be aware of the potential for vulnerability to abuse amongst the service users and their family, and use of the local policy to inform appropriate personnel in line with Safeguarding Vulnerable Adults and Children.
- Participate in the Home Treatment Team (HTT) function, shift pattern is 09.00 – 17.00 or 11.00 – 19.00 and 09.00 -15.00 on Saturday, Sunday and Bank Holidays. The Home Treatment team provides crisis support as well as alternative for admission for older adults with acute mental health
- Ensure details and appropriate records are maintained to demonstrate that contact with people who use the service, and their carers is meaningful and effective. Communication systems are to be fully utilised to facilitate high quality standards of communication with Service Users, Relatives and Carers along with other statutory and voluntary agencies.
