This post is ideal for someone who wishes to pursue a career in Haematology. There will be support and supervision by the consultants, to ensure that competencies are achieved in diagnosis and management of both malignant and non-malignant Haematology.

You will join a team of two other ST3/4 level Haematology trainees, with cross cover for absence. Outpatient and Ward patients should have dedicated cover at all times. No two ST3/ 4 should be absent at once, but if this occurs, the Ward cover also holds the referrals duties. 

All ST3/4 trainees should attempt to attend and present patients at the weekly multidisciplinary team meeting on Tuesday afternoon, and the weekly morphology session.

The successful candidate will be expected to work 09:00am – 5:00pm Monday to Friday on their allocated ward. There is also on call cover involved with the role, you are expected to be first on call one weekend per month supported by consultant.

The haematology service is delivered at WHHT by an innovative, forward-thinking team of 7 Consultant Haematologists (5 full-time/ 2 part time), supported by 6 Clinical Nurse Specialists and 2 registrars from the East of England Deanery. 

The team treats a wide range of malignant and benign conditions, including level 2A malignant haematology patients. Most treatment of malignant haematology patients takes place at the Helen Donald haematology day unit, with some patients requiring in patient chemotherapy administration.

The day unit also offers transfusion service, including for patients with chronic anaemia- nurse led clinics, as well immunoglobulin infusions.

Referrals, day unit and Laboratory Duty:

 Carry the Referrals Bleep 

 Ensure referrals are seen promptly, discussed with the consultant, and shared between you and the relevant consultants if you get too busy. 

 Ensure the online referrals system is checked at least twice daily or delegated to a colleague.

If needed, see the outlying medical patients with haematological issues who are not eligible to come under primary haematology care. 

 Support the nursing staff at the day unit with medical reviews of patients and chemotherapy prescribing, if required

Report blood films*. Supervised by the consultants, to ensure that all blood films are reported within 72 hours (unless flagged as urgent by laboratory, in which case within 4 hours). 

 Attend clinics 

• Perform* bone marrow biopsy on the planned list on Thursdays 

• Administer intrathecal chemotherapy- once completed training and achieved competency

*LAS applicants with no prior experience in reporting blood films, or performing bone marrow biopsy will be trained by the consultant and will only be expected to carry them independently once signed off as competent.

Ward Duty: You will generally be responsible for 8-11 inpatients: mainly patients with post chemotherapy complications and occasionally patients needing to start chemotherapy as in patients. There may also be patients admitted with sickle cell disease crisis and benign haematological disorders such as unexplained lymphadenopathy, and autoimmune haemolytic anaemia. There are three consultant ward rounds a week depending on the attending consultant’s commitments but generally on Mondays, Wednesdays and Friday morning. 

There is also a haematology FY1 or SHO who are based with the haematology team but also have on call commitments with the acute medical take. 

           Carry the Ward Cover Bleep

           Assist and manage the FY1/SHO team on the wards 

           Ensure the standard hospital discharge summaries are done by the FY1/SHO 

           Ensure patients leave with adequate and properly booked follow up plans (next outpatients, next chemotherapy cycle, next day unit review)

On call Duty:

Once the right competencies have been achieved, you will share the on-call rota with the two other ST3+. The usual rota has 3 out of the 5 weekday nights have the ST3+/LAS as first on call (on rotation bases) with the consultant second on call. You are expected to be first on call one weekend per month supported by consultant.

  • Evidence of skills in the management of patients not requiring hospital admission
  • Evidence of ST3-level procedural skills relevant to medical patients

Experience:

  • Evidence of experience in a range of acute medical specialties, with experience of managing patients on unselected medical take, during core training or equivalent
