Community Nursing Team Lead

We are seeking an experienced Community Nursing Team Lead to play a fundamental role in the success of our Community Nursing Teams. This is a key leadership post for an NMC-registered nurse who holds a Specialist Practitioner Qualification (SPQ) and is committed to role-modelling the Trust’s core values and behaviours.
As Team Lead, you will be responsible for the leadership, development and operational management of a designated Community Nursing Team, ensuring the delivery of high-quality, evidence-based, person-centred care to patients, families and carers in the community. You will provide visible, supportive leadership across a seven-day service, managing capacity, prioritising visits and responding effectively to service pressures.
You will supervise, support and develop staff, promoting interdisciplinary working, lifelong learning and professional development. Working closely with the Clinical Lead and key stakeholders, you will support service integration, maintain CQC standards, manage delegated budgets, oversee recruitment and retention, and ensure adherence to policies, performance targets and contractual requirements.

Provide clinical and operational leadership for a Community Nursing Team, ensuring high standards of safe, person-centred care.

Manage day-to-day service delivery, including capacity planning, prioritisation of visits, seven-day service cover, and escalation of risks or pressures.

Lead, supervise and develop staff through appraisal, performance management, clinical supervision, education and training.

Recruit, retain and support staff wellbeing, attendance, and professional development.

Ensure compliance with CQC, organisational policies, clinical governance, safeguarding, health and safety, and risk management requirements.

Manage delegated budgets, resources and staffing efficiently.

Maintain effective communication within the team and with internal and external stakeholders, positively representing the service.

Investigate complaints, incidents and serious incidents, identifying root causes, implementing improvements and sharing learning.

Contribute to service improvement, quality assurance, audits, data reporting and strategic development of community nursing services.

Maintain professional registration, clinical credibility and ongoing personal and professional development in line with NMC standards.

Provide clinical leadership, support and guidance for complex caseloads, triage and evidence-based community nursing practice.
