Surrey and Borders is looking for someone who is passionate about developing and providing high quality, evidence based Crisis Intervention care and treatment to young people with moderate to severe emotional and mental health problems.

Working Location: Unither House, Curfew Bell Rd, Chertsey KT16 9FG

Working Pattern: Monday to Friday - 9AM to 5PM

Job Overview: With resources available within a defined area deliver on the following key area, coordinate and lead team functioning with particular focus on;

·         Management of referrals including delivery of care

·         Supervision of clinical staff across the Surrey-wide service.

·         Provision of  a 7 day/week crisis intervention service, which offers assessments to children, young people and their families presenting in crisis in both local Paediatric wards and CAMHS Community Team Bases

  • Provide urgent specialist mental health assessments for children and young people (CYP) presenting in crisis.

  • Work autonomously to assess, manage, and coordinate care for CYP with complex co-morbid mental health needs.

  • Respond to emergency referrals in collaboration with CAMHS Community Teams.

  • Manage a caseload from referral to discharge or transfer, including medium to high-risk cases.

  • Develop and implement crisis and contingency plans, including safety planning.

  • Coordinate care with paediatric wards, liaison nurses, and multi-agency partners to ensure safe and timely discharge.

  • Contribute to evidence-based practice, audit, outcome monitoring, and service development.

  • Lead on relevant clinical guidelines and protocols within area of responsibility.

  • Participate in delivery of specialist programmes for CYP presenting with deliberate self-harm (DSH).

Clinical Responsibilities

  • Undertake comprehensive clinical assessments of children and young people presenting in mental health crisis, including those with complex co-morbid presentations.

  • Function as an autonomous practitioner, ensuring robust risk assessment, formulation, and management plans are in place.

  • Work collaboratively with medical, nursing, and allied professionals to ensure safe, effective, and timely discharge planning.

  • Coordinate and liaise with Paediatric Wards and Paediatric Liaison Nurses when CYP with mental health needs are admitted.

  • Work closely with intensive intervention teams, paediatric health liaison services, local authorities, and emergency services to develop robust crisis and contingency plans (including My Safety Plans).

  • Actively formulate crisis and contingency plans with CYP and families, incorporating positive risk-taking and clear safety documentation.

  • Be responsible for initial assessments and facilitate onward referrals, warm handovers, or discharge with a signed safety care plan recorded on the electronic patient record.

  • Participate in the delivery of the WISH programme for CYP presenting with deliberate self-harm.

Caseload and Risk Management

  • Manage a defined caseload from referral to discharge or transfer.

  • Hold and manage medium to high-risk cases, ensuring clinical decisions prioritise safety while promoting recovery and least restrictive practice.

  • Maintain accurate, timely, and defensible clinical documentation.

Multi-Disciplinary and Partnership Working

  • Work within a highly experienced multidisciplinary team supporting CYP with serious and complex mental health difficulties in crisis.

  • Collaborate closely with CAMHS Community Teams, intensive intervention services, paediatric services, local authorities, and emergency services.

  • Contribute specialist mental health expertise to joint decision-making in high-risk or complex situations.

Governance, Quality and Service Development

  • Ensure implementation and audit of agreed outcome measures within the team.

  • Contribute to relevant research activity and promote evidence-based interventions across the care pathway.

  • Lead on the development and implementation of clinical guidelines and protocols relevant to the role.

Experience

  • Training others through various mediums
  • Knowledge and awareness of Safeguarding procedures

  • Experience of developing a plan of care to safeguard an adult or child
