Job overview
The LYPFT Learning Disability Service provides specialist, multidisciplinary healthcare for adults (18+) with a learning disability across community settings, respite services, and the Intensive Support Team. We work in close partnership with service users, carers, Adult Social Care, and third sector and independent providers to support people with complex and often unmet health needs, including behaviours that challenge, mental health needs, dual diagnosis, and significant physical health conditions.
We are excited to offer an opportunity for three Band 5 Community Learning Disability Nurses to join our teams. Whether you are a newly qualified nurse at the start of your career or an experienced Band 5 looking for a new challenge, we would love to hear from you.
You will support adults with learning disabilities to live healthier, more independent, and fulfilling lives. You will work with individuals who have complex physical, sensory, communication, and behavioural needs, using your clinical skills and compassion to deliver care that is person-centred, respectful, and empowering.
There are two posts available within the West and South Community Learning Disability Team (CLDT) and one post within the East North East CLDT. The service operates Monday to Friday, 9:00am until 5:00pm.
Main duties of the job
You will be supported to work both autonomously and as part of a multidisciplinary team, developing confidence and independence within your scope of practice.You will manage a caseload, including care coordination responsibilities, ensuring individuals, their families, and carers are supported to understand and implement agreed care and treatment plans. You will build meaningful relationships and work in partnership with a range of health and social care professionals to deliver person centred care.
In this role, you will contribute to a range of clinical pathways undertaking holistic assessments, planning and delivering interventions, and reviewing outcomes. These include Complex Physical Health, Dementia, Positive Behaviour Support, Mental Health, Risk and Vulnerability, Health Desensitisation, Anger Management, and Anxiety Management.

We also provide specialist inpatient services in York and some highly specialised services across the country. Qualifications
Essential criteria
- Advanced Diploma or degree in Learning Disability Nursing. Other branches of nursing would be considered. Registered within the NMC.
Experience
Essential criteria
- Awareness of the Learning disability Nurse role and experience of working with people with a learning disability.
- Experience of working in an MDT.
Skills
Essential criteria
- Skills communicating with a range of service users.
- Driver with access to a vehicle for work purposes.
