Advanced Nurse Practitioner / Advance Clinical Practitioner

Are you looking for an exciting new role in a high performing and supportive team? Perhaps you are ready for your next challenge? We have a fantastic opportunity to recruit an experienced Nurse for an Advance Nurse Practitioner role or Advanced Clinical Practitioner to join the West Berkshire Urgent Community Response / Virtual Ward Team.
The role will provide advanced clinical assessment to patients in their own homes and in a care homes, who require an urgent response to prevent unnecessary hospital admission. The post holder will work as part of a multi-disciplinary team to provide appropriate patient focussed care.
The role will provide leadership and line management to junior staff within the multidisciplinary team, including nurses, paramedics, and support workers, and clinical supervision and training/ education to the nurses, paramedics, and support workers, as required.
You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards. You will ensure good communication, and effective processes and procedures are maintained across the service.
The post holder will work 37.5hrs per week, 7 days a week, between the hours of 8am – 8pm, which will include weekends.
The role is based at Hillcroft House, West Berkshire Hospital

To practice at an advanced level of nursing, working as an autonomous practitioner within the multidisciplinary team including admitting to and discharging patients from the Urgent Community Response Service in collaboration with the Community Geriatrician and GP.

As an experienced advanced nurse practitioner, you will take responsibility and be accountable for the clinical assessment, diagnosis, treatment and management of complex patients considering local and national guidelines.

The postholder is expected to liaise effectively and professionally with all levels of staff, external organisations and the public, dealing with confidential information in an appropriate manner.

As an Independent Prescriber, prescribe and review medications in accordance with NMC guidance, local and national policy guidance.

To provide leadership and line management to the Band 7 nurses and therapists and the Band 3 & 4 healthcare/multi-therapy assistants.

Ensure accurate, contemporaneous documentation and care plans are maintained accordance with the NMC, information governance and local / national guidance.

Work collaboratively with colleagues and develop nursing and care delivery policies, identifying how policy will impact on practice and service delivery.

- Registered General Nurse.
- Post registration Advanced Clinical Study e.g. diagnostic reasoning
- Non-medical Prescriber Level 7.
- Current NMC Registration.
- Evidence of continuing professional development
- To have undertaken study at masters level or willing to do so.
- Experience of teaching, mentoring, supervising
- Qualification in teaching and assessing.

Previous Experience

- Substantial post registration experience and two years’ experience working at band 7 or equivalent.
- Experience of complex case management.
- Extensive knowledge of older people’s and long-term conditions agenda.
- Demonstrate a high level of clinical skills.
- Demonstrate an in-depth knowledge and experience of clinical examination, history taking, pharmacology, pathophysiology, etc.
- Experience of instigating and interpreting investigations and initiating appropriate plans of care for patients.
- Experience of promoting self-care.
- Cultural awareness.
- Experience of multi-agency working across service interfaces.
