Patient Flow Coordinator

We have an exciting opportunity within our community inpatient unit, at Wokingham Community Hospital for a clinical administrator, called a Patient Flow Co-ordinator. The post holder will be an integral member of the multi professional team and will support the patient flow through the admission and discharge process.
There are two wards within its Inpatient Unit, caring for adults of any age with a wide mix of health care needs. A proportion of the patients will require continued support after discharge, and the post holder is the link between the patient and relatives, the team and the ongoing services required on discharge.
The post holder will attend MDT and allocation meetings ensuring that all referrals are placed in a timely fashion, maintaining a seamless referral process and communicate with patients, relatives, and external services. Also, the post holder will assist in the coordination of care for safe transfers when a patient is referred into the service from acute hospitals or their homes.

The Patient Flow Co-ordinator will assist in facilitating admissions and discharges, reviewing and processing referral documents and communicating with external services, like the In Reach Team, Intermediate Care, Community Health and Social Services.
The Patient Flow Co-ordinator is an administrator, and a part of the Integrated Therapy/Flow Teams, where clinical knowledge is beneficial from NHS, general practitioners, or social service backgrounds. They will be the point of contact for our service for our patients, relatives and professionals.
