Are you passionate about making a meaningful difference in the lives of young people and their families? We are looking for a skilled and compassionate Mindworks Community Clinician to join our dynamic CYPS Mindworks Hub team.
 
In this rewarding role, you will play a vital part in delivering timely mental health support for young people with moderate to severe emotional and mental health needs, using your clinical expertise to assess, triage and provide effective, person-centred care.

Main duties of the job

The post holder will be a member of the multi-disciplinary specialist team, providing services to children and young people 6-18 years and young people in transition to adult services with mental health needs, emotional and behavioural problems whilst also providing leadership to our Mindworks community team.

The main responsibilities of the post holder will be providing assessments and treatment within our Mindworks Community Teams in North Surrey either individually or with a member of the multidisciplinary team depending on the complexity of the referral, both in clinic and community settings. The post holder will be able to offer a range of therapeutic interventions appropriate to the care pathway the young person is on.

As part of a busy CYPS Directorate and alongside our Alliance Partnership including local voluntary sector organisations, you will be supporting the team in leading with the implementation and embedding of the THRIVE Framework across our newly created Mindworks Community Team in Surrey.

The post holder will work closely with the Mindworks Alliance including School Based Needs, Surrey Well Being Partnership and other agencies during office hours, responding  to referrals of young people presenting in need.

Experience

Essential criteria
  • Demonstrable post-qualification experience of working in the community with young people with moderate to severe mental health difficulties and their families.
  • Ability to Undertake: Mental Health Assessments, Risk Assessment and Implement Risk management Plans and outcome measures.

Qualifications

  • Relevant professional qualification and registration in Nursing, Social Work or Occupational Therapy (NMC, HCPC or SWE)
