Main duties of the job
Surrey and Borders is looking for someone to be a core member of the multi-disciplinary CYPS Community Learning Disability Team, participating in the provision of initial assessments assessments, care planning and treatment of young people with moderate to severe Learning disabilities and associated mental health need.
Job Purpose
· To provide timely assessment of children and young people who are referred, including offering discovery conversations which may include deliberate self- harm or injury, safe guarding needs etc and provide timely advice and support including signposting post assessment and any onwards referral to support needs
· To provide support, advice and education to the schools, social care and any other professionals to build knowledge and skills of staff within these settings in the care and treatment of children and young people.
· To work individually and jointly within the Mindworks alliance to provide a specialist, assessment and treatment service to children, adolescents and their families.
· Offer evidence based interventions to continue supporting children and young people post assessment including Positive Behaviour Support.

This role is working within i-access, a successful, countywide drug and alcohol treatment service for Surrey residents delivering tier 2, 3 and 4 substance misuse interventions across 3 main bases and 25 satellite locations. i-access is run collaboratively by SABP, PH / SCC and Via under a cooperative agreement which is one of partnership and transparency and contributes to Surrey Heartlands wider strategic ambitions.
Detailed job description and main responsibilities
Key Responsibilities
The post holder will work closely within the Mindworks alliance during office hours, responding to emergency referrals of young people presenting in crisis:
· The post holder will work as an autonomous practitioner, undertaking clinical assessment of children and young people who may have a complex co-morbid learning disabilities with associated mental health needs working alongside medical, nursing and other professional colleagues to ensure that the most effective care and treatment is provided resulting in safe care to meet identified risk and need formulation.
· Work closely with other Mindworks Services, Social care, On Call Managers, Paediatric Liaison Nurses, Schools etc,to offer support and guidance.
· To work within a highly experienced multi-disciplinary team involved in the assessment and treatment of children and young people with learning disabilities and associated mental health needs.
· To manage own caseload from referral to discharge/transfer, holding medium to high level of formulated need and risk young people.
· Ensure that Goal Based Outcomes are implemented and audited. Contribute to relevant research and ensure that interventions on the pathway are evidence based.
Other duties:
· To actively formulate a robust My Safety Plan and/or Crisis and Contingency Plan with young people and their families inclusive of risk formulation, management and positive risk taking.
· Contribute to the development and implementation of clinical guidelines, protocols relevant to sphere of responsibility.
· Ensure accurate, relevant and timely record keeping is completed as per professional guidelines and trust policy on system 1
· Ensure young people and their families are offered carers assessments
· Attend any relevant professional’s meetings to support young people within our service.
· Ensure safe guarding referrals are completed in a timely manner and relevant documentation completed as per local policy
· To ensure Key Performance Indicators are completed and updated should needs change.
· Access supervision and participate fully to discuss young people you are working alongside and any support you may need, safe guarding concerns, training etc.

Essential criteria
- Degree level qualification in Nursing, Social Work or Occupational Therapy
Experience
Essential criteria
- Two year’s post qualification experience of working in the community with young people with moderate to severe learning disabilities and associated mental health needs
- Ability to Undertake: Assessments including mental health needs, risk formulation and implement risk formulation plans and outcome measures
