West London NHS Trust  provides a full range of mental health, community  and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

Ealing Cognitive Impairment and Dementia Services, within the Older People’s Mental Health Service, based at Acton and Greenford in EALING.

We are looking for dedicated, enthusiastic nurses who are highly motivated and caring professionals who can provide the best possible care with compassion to our patients.

 

The successful candidate will have post registration experience within Dementia or community Care, knowledge and experience of older adults with mental health needs or be able to demonstrate your transferrable skills, resilience and commitment through your application and at interview

Prif ddyletswyddau'r swydd

The post holder will provide specialist assessments, reviews and nursing interventions to service users accessing the Cognitive Impairment & Dementia Service (CIDS). The post holder will ensure the successful implementation and roll out of care for dementia patients

 

Main duties include but are not limited to: 

·       Specialist assessment of individuals and their families

·       Assessment and management of ongoing risks as identified during the assessment, ensuring that this is done in collaboration with service users and carers. Provide supervision of junior staff and trainees where appropriate.

·       Work as a core member of an MDT.

·       Act within the NMC Code of Conduct at all times.

·       Deliver care under the Care Programme Approach and work in partnership with individual service users and their families and carers to ensure the delivery of the care plan.

·       Administer and monitor medication, including depots, prescribed to individual service users, providing information and advice, within NMC guidelines and the Trusts Medicines Code.

·       Broaden research and development skills through participation in local audit and research projects.

Qualifications and Training

Experience

  • Experience of working with patients and their carers with complex health and social care difficulties in the designated care group.
  • Experience of supervising staff.
  • Experience of managing clinical caseload will include carrying out initial assessments, treatment efficacy reviews and risk mitigation assessments leading to crisis prevention
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health difficulties.
  • Experience of working consultatively with professionals.
  • Experience of conducting a range of clinical assessments including risk assessments.
  • Experience of partnership working within and across various statutory and non-statutory teams and agencies
  • Previous experience of working with carers and independent / voluntary sectors.
