Community Nurse

Band 5 Nurses work with a defined caseload of service users and usually as part of a multi-disciplinary team, providing assessment, care planning and care delivery, often (but not always) under the supervision of a more senior practitioner. This will include the provision of highly skilled care, psychological interventions, group work, clinical assessment, risk assessments / risk management, and promoting recovery and inclusion.
Band 5 posts within a community setting will be required to: work autonomously, manage caseloads, undertake initial assessments and carry out home based treatment interventions.
Please note as this is a temporary role we cannot accept applications from newly qualified nurses, or those requiring a preceptorship.

The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country.
