Job overview
An excellent opportunity has arisen to work with Community Post covid MDT team in Ealing and Hillingdon Borough and also support triage. The MDT Is supported by GP, Psychologist and Respiratory physiotherapist. This is a permanent post for 30 hrs a week. The Occupational Therapist will be supporting vocational rehab, fatigue - brain fog management, breathlessness management, MDT case reviews and undertaking triage, Clinics will be based in Ealing and Hillingdon along with some telephone appointments including use of digital app and triage.
The service is now renamed as Post Infection Conditions Service formerly known as Post Covid rehabilitation service. The service works closely with ICB and with other boroughs across North West London, supporting triage and participating in regular NWL borough based IST. Clinical support is available from Senior Occupational therapist and also from service lead. There is access to local and borough wide training and shadowing of medical clinics as part of CPD.
Main duties of the job
To provide a highly skilled occupational therapy assessment, education, advice and treatment to adult patients referred to post covid rehabilitation and support pulmonary rehabilitation and other community services if required and work to the standards set by BAOT and HCPC.
To Triage / screen and assess referrals at the Single point of access, including gathering medical history from GP records/referrer and identifying if appropriate or not; establish priorities and assign to appropriate pathways.
To participate in weekly Multidisciplinary team meetings with other colleagues for complex case reviews.
To undertake patient management/treatment in a variety of settings utilising a range of methods to do this e.g. individually or group therapy/rehabilitation interventions, face to face, virtual and app-based technology .
To provide assessment and treatment of Post covid patients and chronic respiratory conditions that are referred to the service for fatigue, Cognitive issues, breathlessness vocational rehab support.
Participate and lead in audits and service development.
Maintain accurate documentation on electronic patient records including discharge letters, clinic updates letters and clinic notes.

Detailed job description and main responsibilities
To assess, diagnose and develop individual care/treatment plans for a range of conditions, some of which will be highly complex or will be for patients with complex clinical and social needs.
To be responsible for reassessing patients care as treatment progress and alter treatment programme as appropriate.
To undertake highly specialist assessment and treatment of patients who may present with long term multiple conditions or with complex presentations of fatigue, brain fog, low mood.
To be able to prioritise on needs-based care approach.
To demonstrate excellent communication within MDT and other stakeholders.
To participate in local and cross borough Inservice training, deliver training and promote the service
To keep accurate up-to-date records on patient care and equipment provision, which comply with quality standards of Trust/Professional/Service policies, meet legal requirements and that can be shared with colleagues and carers when appropriate.
To ensure a high standard of clinical and professional practice.

Essential criteria
- Degree or Diploma in Occupational Therapy
- Evidence of relevant CPD
- Health Care Professionals Council Registration

Experience
Essential criteria
- NHS Experience in acute and community in core areas of rotation eg medical, surgical, orthopaedic, neurology, care of elderly
- Experience of leading Audit
- Experience of supervision / teaching
- Understanding of clinical governance

Skills and knowledge
Essential criteria
- Clinical triage and managing long term complex conditions
- Sound assessment skills with advanced clinical reasoning and analytical ability especially with multipathology, fatigue management, vocational rehabilitation and brain fog
- Ability to critically appraise information/journal articles
- Keyboard skills and ability to access information via the internet
- Evidence of self-directed learning and reflective practice
- Ability to work independently and prioritise workload and pressures
- A sound theoretical knowledge of manual dexterity skills and therapeutic manual handling techniques
- Presentation skills and the ability to pass these skills and knowledge to others
- Knowledge of developments within NHS community and occupational therapy profession
- Good understanding of role of the Occupational Therapist in rehabilitation as well as the role of other professionals in MDT
