Band 6 Nurse You will be working within the Urgent and Crisis and Home Treatment area of the Gleadless and Heeley Neighbourhood Mental Health Centre, working shifts of 8am – 8pm across 7 days per week on a rota basis. You will join an exciting new team, one of the first 6 pilot sites across the UK to launch to the Neighbourhood Mental Health Model of care. The aim is to establish a coherent, functionally integrated team, working with the VCSE, social care and the local community to deliver high quality mental health care for all the people of Gleadless & Heeley aged 18 or over. The Centre will embrace the 10 key principles highlighted for the NMHCs: Trusted Relationships Continuity of Care Open Access All Means All Co- Produced Citizenship & Belonging Close to primary care and system partners Neighbourhood based Freedom, Autonomy & Choice Do not harm.

General requirements
- Maintain NMC registration and adhere to professional guidelines and code of conduct
- Work within the framework of relevant policy and legislation both local and national
Principal duties
Clinical
- Triage new referrals to the service
- Carry out comprehensive assessments of an individual’s health and social care needs in a variety of settings and agree the most appropriate course of action
- To assess, evaluate and manage clinical risk
- To formulate a plan of care based on identified need
- Deliver evidence based crisis interventions as appropriate
- Review effectiveness of care plan at regular intervals
- To work collaboratively with service users, carers and partner agencies
- To coordinate the clinic work of a day by day basis
- To maintain computer held care records in an accurate and timely fashion
- Undertake all work in a culturally sensitive manner
- Act in accordance with relevant policy and legislation.
- Develop and support effective working relationships with inpatient, community and voluntary organisations.
- Ensure timely and supported step down from acute inpatient setting and provide preventative intervention to avoid unnecessary acute admission.
- Support admissions for service users within the Heeley + PCN to access the onsite hospitality beds to avoid further escalation of crisis.
Supervision
- Regularly review own clinical work with supervisor to promote efficacy and contribute to the overall management of the HTT & NMHC
- Deliver supervision and facilitate annual PDR with more junior members of staff
- Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve level of competence and effective contribution
Education and Training
- Act as mentor to learners, keep up to date with relevant course and curriculum developments, and maintain ‘sign off’ status
- Participate in training relevant to role, including training needs for the team.
- Promote insight and understanding of mental health issues to service users, carers, and other agencies
- Contribute to audit, research and evaluation as appropriate
Service Development
- Contribute to service development and review of development of the NMHC in line with national development of the pilots.
