An opportunity has arisen for the right person to join the Dementia and Delirium Team for 4 days a week based at Newham University Hospital, Barts Health NHS Trust. 

We are seeking to recruit a Dementia Specialist Nurse / Occupational Therapist for 18 months, to support the delivery of the Trust Dementia Strategy, to help improve the care and experience of those living with dementia or delirium and their carers. The role will also support the National Dementia strategy by identifying patients living with dementia and other causes of cognitive impairment, to prompt appropriate referral and follow up to prevent re-admission.

We are looking for a nurse / occupational therapist with creative flair who is experienced in working with people living with dementia. You will be enthusiastic, innovative and forward thinking with a passion for improving the lives of people living with dementia and/or delirium.

We are working towards improving services in line with the national and local objectives in dementia and delirium care, and are looking for someone who is willing to go the extra mile to ensure our patients get excellent, person centred care.

The successful candidate will be required to carry out clinical assessments on those identified with a cognitive impairment. The candidate will work closely with the multidisciplinary teams on the wards to ensure the patient is at the centre of the service we deliver.

The post holder will work with the dementia teams across the trust to support the implementation and evaluation of a semless Dementia Care Pathway, ensuring that patients  receive the highest standard of clinical care, treatment and support throughout the hospital journey and beyond.

 

They will specialise in the care of people with dementia and delirium in the acute setting and will demonstrate a high level of expertise, providing specialist advice and support to ward teams, patients and their carers.

 

They will play a key role in the delivery of the Trust dementia and delirium education programme,  providing training and evaluating training programmes. The post holder will actively undertake clinical audits and research within the specialist area.

 

Together with the Clinical Lead for Dementia and Delirium, the post holder will contribte to the embelling of KPI targets and  NICE quality standards across the Trust, and  the implementation of the objectives within local and National Dementia Strategies.

 

The post holder will influence the ongoing development of clinical practice within delirium and dementia care practice, undertake research and set standards of care within the service, including the development of policies, protocols, guidelines and service developments in collaboration with multi-disciplinary colleagues.
