An exciting opportunity has arisen for a passionate Community Nurse to join the Bollington, Disley and Poynton Team as B5 Community Staff Nurse

Our team is based at Priorslegh Medical Centre in Poynton and provides care across the Bollington, Disley and Poynton area, supporting a local population of approximately 33,000 residents. We are a committed, friendly and forward-thinking multidisciplinary team with a shared vision to deliver the right care, at the right time, for the first time, helping to improve the lives of our local residents.

We are looking for a motivated nurse with a positive and proactive approach who can work collaboratively with colleagues across community nursing, therapy services and primary care. The successful candidate will work closely with the therapy team and GPs from the Middlewood Partnership to support safe, coordinated and person-centred care.

This is an excellent opportunity for an experienced community nurse who is keen to further develop  clinical skills, support service improvement and make a positive difference to patients, families and colleagues within the Bollington, Disley and Poynton care community.

Main duties of the job

• To contribute and support the delivery of high standard of effective nursing care, as part of a nursing team, in support of and under the direction of a registered professional, to be an autonomous practitioner who is responsible for the holistic assessment of both complex and simple care needs, the development, implementation and evaluation of care and delegation of care where appropriate to other members of the team.
• To carry out assigned tasks, including direct nursing care and administrative duties as delegated by the nurse in charge ensuring that patients / clients receive safe, individualised and effective care which supports health & wellbeing and promotes self- care where appropriate and possible
• To deliver physical, psychological and emotional care to patients in line with the patient care plan and under the direction and supervision of the registered case manager in the patient’s home or community-based facility.
• To report information to trained staff on progress of patients and any variance/change in patient/carers care management in a timely manner. To complete patient records in line with local record keeping standards and protoco
