We are looking for a compassionate and motivated Senior Perinatal Community Practitioner to join our Specialist Perinatal Mental Health Team. Based primarily within the York and Selby locality, you will provide specialist assessment, intervention, and support for women and birthing people experiencing moderate to severe mental health difficulties during pregnancy and up to the baby's first year.

As a senior team member, you will work closely with families, maternity services, primary care, and partner agencies to deliver person-centred care. You will also contribute to service development, promote best practice, and support colleagues in providing excellent care.

Although mainly based in York and Selby, the role requires travel across the Trust footprint to support service delivery, attend meetings, training, and development opportunities.

We are committed to staff development and offer access to specialist perinatal mental health training, leadership programmes, and ongoing professional development to support career progression.

Applicants must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework at the point of application. 

 

Main duties of the job

As a Senior Perinatal Community Practitioner, you will play a key role in delivering high-quality specialist mental health care to women and birthing people experiencing moderate to severe mental health difficulties during the perinatal period.

Key responsibilities include:

  • Undertaking specialist assessments 
  • Developing, implementing, and reviewing evidence-based care plans 
  • Delivering specialist therapeutic interventions and coordinating care within a community setting.
  • Providing consultation, advice, and support to professionals across maternity services, primary care, health visiting, social care, and voluntary sector organisations.
  • Working collaboratively with women, birthing people, families, carers, and partner agencies to promote positive outcomes for both parent and infant.
  • Contributing to safeguarding processes and ensuring the safety and wellbeing of service users, infants, and families.
  • Acting as a lead professional where appropriate and maintaining responsibility for a defined caseload of service users with complex needs.
  • Ensuring care is delivered in a trauma-informed, culturally sensitive, person-centred, and inclusive manner that values diversity and promotes equality.
  • Contributing to service development, quality improvement projects, audit activity, and the implementation of evidence-based practice

We are looking for:

  • A qualified mental health professional (RMN, Social Worker, Occupational Therapist) with current registration and at least 18 months post-qualifying experience.
  • Strong communication, assessment, and risk management skills.
  • Significant post-registration experience, ideally in perinatal, maternal, or adult mental health.
  • A compassionate, non-judgmental approach and a commitment to holistic, family-focused care.

Qualifications

Essential criteria
  • Current registration with an approved professional body
  • Evidence of continued professional development
  • For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education. Must be achieved within agreed timescale.
  • 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework (at point of application)
  • Recognised sign-off mentor, clinical educator or equivalent. Must be achieved within agreed timescale.
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
Desirable criteria
  • Leadership or management qualification
  • Post-graduate qualification in a relevant area

Experience

Essential criteria
  • Significant experience working with people with mental ill health in a care environment.
  • Working collaboratively with service users and their families/carers
  • Providing clinical supervision to individuals or groups as professionally appropriate.
  • Mentoring or facilitating students on practice placement
  • Working in a multi-disciplinary team
  • Quality improvement activities

  • Leadership or management experience

Essential criteria
  • Demonstrable knowledge to post-graduate level of evidence-based practice in caring for patients inthe designated field.
  • Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act).
  • Care Programme Approach and its application in practice
  • Detailed understanding of safeguarding and its application in practice
  • Clinical Risk Assessment and Management and its application in practice.
