Community Psychiatric Nurse - Rough Sleeping Team
Band 6
West London NHS Trust provides a full range of mental health, community and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.
We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

The Trust successfully bid to establish a Rough Sleeping Team and we are now recruiting staff to a Multi - Disciplinary Team. An opportunity exists for a CPN to join this Team and play a key part in its development and response to the needs of rough sleepers who have mental health difficulties and or serious physical health challenges. The service works across the tri-Borough area of Hammersmith & Fulham, Ealing and Hounslow and works closely alongside our principal partners , St Mungos and Thamesreach Outreach Services.

This is an exciting development providing a much-needed service and in addition to your established professional skills you will be a good communicator with a degree of flexibility and willingness to work with a number of agencies. The Team, in addition, includes a Consultant Psychiatrist, Social Workers and an OT.
The post involves working shifts which enable us to access service users effectively in order to assess mental and physical health needs , engage successfully and pursue reconnection with health and appropriate accommodation services.

- Evidence of continuous professional development
- At least 2 years professionally qualified
- Completion of Mentorship Course/ENB equivalent

Experience

- Experience of working alongside people with complex health problems in the designated care group
- Experience of supervising staff
- Experience of managing a caseload
- Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
- Experience of working consultatively with professionals
- Experience of conducting clinical assessments including risk assessments
- Experience of partnership working within and across various statutory and non statutory teams and agencies
- Experience of working within the CPA process
- Previous experience of working in the community
