Are you an enthusiastic and motivated nurse with an interest in tissue viability, wound care and pressure ulcer prevention? We have an exciting opportunity for a registered nurse to join our Community Tissue Viability Team as a Band 6 Tissue Viability Nurse on a 13-month fixed-term basis to cover maternity leave.

This role offers an excellent development opportunity for a nurse wishing to expand their specialist knowledge and skills within tissue viability whilst working as part of a supportive community team.

We are looking for an individual who is:
•    A Registered Nurse with current NMC registration.
•    Experienced in wound management and pressure ulcer prevention.
•    Passionate about improving patient outcomes and promoting evidence-based practice.
•    An effective communicator with strong interpersonal and teaching skills.
•    Able to work independently and as part of a specialist team.
•    Committed to continuous professional development.

Base- Guildford and Waverley Adult Community Health Services, based at Milford Hospital

Main duties of the job

As a Tissue Viability Nurse, you will support the delivery of a high-quality specialist service across the organisation. Working alongside the Tissue Viability Team, you will provide expert advice, education and clinical support to healthcare professionals, patients and carers in relation to wound management, pressure ulcer prevention, skin integrity and continence-associated skin damage.

You will contribute to the assessment and management of complex wounds, support quality improvement initiatives, participate in audit and incident review processes, and assist in the development and delivery of training programmes.

Key Responsibilities
•    Provide specialist advice and support relating to tissue viability and wound care.
•    Assess, plan and evaluate care for patients with complex wound care needs.
•    Support the prevention and management of pressure ulcers and other skin integrity concerns.
•    Deliver education and training to clinical staff across the organisation.
•    Contribute to audits, investigations, service developments and quality improvement programmes.
•    Work collaboratively with multidisciplinary teams to promote best practice and positive patient outcomes.
•    Participate in the monitoring and evaluation of clinical standards relating to tissue viability.
