We are recruiting to our team of 21 rotational Band 6 pharmacists, offering an exciting opportunity to gain experience across a wide range of general and specialist clinical areas, including surgery, respiratory, cardiology, medical admissions, neurology, stroke, mental health, medicines information and technical services.
We're committed to supporting your professional development and offer the opportunity to undertake a fully funded two-year Postgraduate Diploma in Clinical Pharmacy Practice, alongside the option to study for an independent prescribing qualification. You'll benefit from structured training, ongoing support and the opportunity to learn from our experienced specialist pharmacists.
As a valued member of the multidisciplinary team, you'll play an important role in delivering safe, effective and patient-centred pharmaceutical care, helping patients get the best outcomes from their medicines.
This role also includes participation in the Emergency Duty Commitment (EDC) rota, alongside the usual pharmacy service rotas.

Main duties of the job
As a rotational Band 6 pharmacist, you will deliver patient-centred pharmaceutical care across a variety of clinical settings, working alongside experienced pharmacists and multidisciplinary colleagues.
Your key responsibilities will include:
- Providing safe, effective and evidence-based pharmaceutical care, identifying and resolving medicines-related issues.
- Supporting the appropriate use of medicines in line with best practice, local formularies and Trust policies.
- Undertaking medicines reconciliation, clinical screening of prescriptions and discharge summaries, and providing medicines advice to patients, carers and healthcare professionals.
- Contributing to the prevention of medicines-related harm and improving patient safety.
- Supporting the delivery of pharmacy services through dispensary, ward, weekend, late shift and on-call rotas.
- Providing professional supervision and accuracy checking within the dispensary.
- Participating in medicines information, controlled drug management, quality assurance and departmental audits.
- Supporting patients receiving oral chemotherapy, including appropriate counselling and BOPA accreditation.
- Participate in dispensary, ward, weekend, late shift and on-call rotas, including Emergency Duty Commitments (EDC), to support the safe and effective delivery of pharmacy services.

- Participate in the dispensary, ward, weekend, late shift and on-call rotas contributing to service delivery. Adhere to all established procedures and complete all necessary records.
- Clinically screen prescriptions and discharge summaries to ensure the safe, legal and effective use of medicines in line with the local formulary and departmental policies.
- Record all major interventions made.
- Medicines Information – deal with routine enquiries that arise on the ward and in the dispensary. Refer on complex queries to the medicine information team.
- Provide advice and information to patients/carers to ensure compliance with treatment.
- Ensure that a full medicine reconciliation is undertaken for each patient using appropriate sources of information.
- Dispensary - provide professional supervision of the dispensary area on a rotational basis. Provide final accuracy checking of dispensed items.
- Check issues of controlled Drugs and participate in departmental audits of CDs.
- Handout oral chemotherapy prescriptions and counsel patients as prescribed on chemocare, including BOPA accreditation.
- Provide input to Quality Assurance (QA) and quality management systems for the Pharmacy department and Trust under the supervision of the QA team.
Person specification
Education / training / qualifications
Essential criteria
- Masters Degree in Pharmacy (or equivalent: BPharm or BSc [Pharmacy])
- Successful completion of the trainee year and registration exam
- Registered with General Pharmaceutical Council for Pharmacists (GPhC)
Desirable criteria
- Member of Royal Pharmaceutical Society
- Completion of the Post Qualification Foundation Pharmacist Programme
- Completion of the Certificate Year of Clinical Diploma or equivalent
- Completion of the Postgraduate Clinical Pharmacy Diploma or other recognised postgraduate qualification to a similar standard suitable for this role
- Qualified Independent Prescriber (or willing to complete)
Experience

- Experience of medicines supply within the NHS
- Experience for recognising opportunities for change, innovation, and quality improvement
- Experience of using digital technologies to support improved patient safety and outcomes
- Experience of audit and quality systems E A + I
