Job overview

Integrated Discharge at Imperial College Healthcare NHS Trust has as an exciting opportunity within the Charing Cross Integrated Discharge Team. We are looking for an experienced, self -motivated, passionate and dedicated individual  to join our Hospital Discharge team within the acute setting.

The post holder will be embedded within the hospital flow structure and will play a pivotal role in efficiently managing and overseeing discharges across inpatient wards. Within this role, you will work alongside the existing discharge team lead, to manage the team day to day, manage your own complex case load and be an expert point of contact for complex discharge issues 

As well as being a critical and highly valued member of Team Charing Cross, you will also be a part of the wider trust’s Integrated Discharge Leadership Team where you will work towards making trust-wide improvements to hospital discharge and flow with lots of opportunity to contribute to or lead on Quality Improvement projects and support colleagues on other sites. Our aim is to improve hospital flow, prevent delays, improve patient experience of hospital discharge and work collaboratively with internal and external stakeholders.

Main duties of the job

  • To lead and manage the complex discharge team on a day-to-day basis, as part of the Integrated Discharge Leadership team, providing clinical leadership, support and guidance. 
  • To take day-to-day responsibility for the leadership, delivery and management of the complex discharge service. 
  • Plan and organise this efficiently and effectively with regard to patient management and use of time.
  • To ensure that all staff within the team have appropriate support for their practice and continuing professional development through team meetings, review, supervision, 
    appraisal, training and development. 
  • To review and update the documentation relating to discharge planning to improve the quality accuracy and consistency of information recorded and to undertake annual documentation audits. 
  • To provide education to clinical and nursing staff regarding the remit of the Discharge team. 
  • The post holder is required to work out of hours (specific rota details held locally within departments).

Essential criteria
  • Degree or equivalent level of ability gained through NHS experience in a management setting
  • NMC Registration
  • Hold and NMC approved mentorship qualification
  • Evidence of ongoing professional development

  • Relevant post graduate qualification or equivalent experience
  • Leadership development

Experience

Essential criteria
  • Be able to demonstrate situations where effective leadership and management skills have been used
  • In depth knowledge of National Discharge Guidelines
  • Experience in complex discharge planning working with clinical, operational and external partners
  • Significant operational experience working as a senior nurse or equivalent within an acute hospital, intermediate care or community
Desirable criteria
  • Experience in developing and managing high quality strategic direction and delivery
