Job overview

An exciting opportunity to join  our Home First Rehab & Reablement team.  Our service covers the whole of Salford facilitating clients discharge back into their own home. Our vision for Home First is ‘People are in the right place, at the right time - meaning nobody spends more nights away from home than is absolutely necessary’

We are seeking for caring and compassionate people who enjoys interacting and assisting people in their own homes and assisting them with their daily living activities.  You will be an excellent team player and also possess excellent communication skills and must be able to work on your own initiative.  Candidates must be warm and friendly, with a strong work ethic, a positive attitude and a genuine passion for providing high quality care and support for the people of Salford.  

Due to the role working in the community a Full Driving Licence and the use of own vehicle is essential.

The Home First Reablement Service operates 7 days a week from 8am to 10pm and various shift working is available.  The successful applicants will work on a rota that is provided in advance to cover shifts that includes weekends and Bank Holidays.

Unfortunately, this role does not supply for sponsorship.

Main duties of the job

To provide support and person focused re-ablement as required to help ensure the efficient and effective running of the service.

To provide excellent customer service and establish and maintain communication between individuals, relatives, carers and the multidisciplinary team.   To provide holistic patient care, correctly undertake tasks that have been delegated and to record information accurately and pass it on to the relevant people in the team in a timely manner.  Duties may include:

  • Assist the individual with personal care tasks, such as washing, dressing, meal & fluid prep, toileting and administering/reminding medications
  • Assist in the safe lifting, transferring, repositioning and movement of the individual
  • To support individuals to live independently in their own homes and help achieve their own outcomes
  • Observe, monitor and record clients physical and emotional well-being, and promptly report any changes to senior staff 
  • Record all visits and care on the Electronic Patients Record (EPR), Liquid Logic and in the Patients' personal folder 
  • Encourage clients to participate in Social and recreational activities
  • Attend relevant training for the post
  • Attend Daily Huddles

Person specification

Knowledge Training & Experience

Essential criteria
  • Demonstrate a knowledge of reablement
  • Previous healthcare experience
  • Experience working in a team
  • Commitment to undertake Continuous Professional Development (CPD) through training opportunities.
Desirable criteria
  • Previous experience in acute settings.
  • Previous experience in community reablement team

Qualifications

Essential criteria
  • NVQ Level 2 and willingness to undertake further study to develop own skill and gain a NVQ Level 3
  • Level 1 Literacy and Numeracy (GCSE English and Maths Grade D-G/3-1) as a minimum
  • Care Certificate or willing to undertake on the job training
Desirable criteria
  • NVQ Level 3

Skills & Abilities

Essential criteria
  • Good communication (written and verbal) skills
  • Good organisational skills and ability to prioritise
  • Proficient in use of IT
  • Ability to travel to different geographical locations across the city of Salford and Trust footprint in line with service needs
Desirable criteria
  • Evidence of the acquisition of relevant skills associated with the role i.e. physical observations.

Driving License

Essential criteria
  • Hold a full driving license and have the use of own vehicle
