An exciting opportunity has arisen for a Band 6 Senior Nurse for Urgent Community Response (UCR) to join our Adult Community Nursing Service. Working across Guildford and Waverley, you will deliver urgent, unplanned and planned nursing care to adults in their own homes, helping to prevent unnecessary hospital admissions and promote independence. You will use advanced clinical skills to assess, plan and deliver evidence-based care for patients with complex health needs, long-term conditions and those requiring end of life care. Working as part of a multidisciplinary team, you will contribute to the delivery of the UCR two-hour standard while supporting the wider community nursing caseload. Main duties of the job

As a UCR Nurse you will provide high-quality, patient-centred nursing care to adults in their own homes, undertaking holistic assessments, physical examinations and clinical interventions. You will respond to urgent and unplanned referrals, prioritising visits according to clinical need and supporting delivery of the  UCR service. Responsibilities include medicines management, complex wound care, catheter management, palliative and end of life care, and maintaining accurate electronic patient records using EMIS. You will work closely with GPs, community services and acute services to coordinate care, promote independence and avoid hospital admission. The role also includes supporting service improvement, clinical audit, supervision of junior staff and students, and acting as a clinical role model within the community nursing team.

Shifts cover the service hours of Monday to Sunday between 8am and 8pm. A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Education / Training / Qualifications

Essential criteria
  • Registered Level 1 Nurse with current Nursing and Midwifery Council registration
  • Band 6 experience in the community setting
  • Practice Supervisor and Assessor (to be completed within 1yr of post)

Knowledge / Skills, Ability and Experience

Essential criteria
  • Ability to prioritise visits in accordance with patient need and to organise workload without direct supervision
  • Experience prioritising, and allocating assessments in accordance with patient clinical need and individual competencies
  • Broad range of clinical skills relevant to Community Nursing, - including but not limited to Compression Bandaging, Dopplers, Venepuncture, Male & Suprapubic catheter, Syringe drivers and Palliative care Symptom Management.
  • A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010
