Job overview

To provide a fully comprehensive administration function to support the day to day running of Skylark Rehabilitation Unit.

Skylark Rehabilitation Unit is a 30 - bedded facility that is located in Yate (South Gloucestershire).  The unit is one of a number of community units providing support and active rehabilitation for those people being supported on Discharge to Assess Pathway 2. 

Following a stay in the acute setting, the ethos for all people being discharged is 'Home is Best' and a large number of people are supported to return home utilising the robust and established Discharge to Assess Pathway 1. 

For those people that are unable to return directly home (for a number of different reasons including safety concerns) and are identified as having on-going rehabilitation needs, they are considered to 'step down' into a Pathway 2 Rehabilitation, Reablement and Recovery bed at the Skylark Rehabiltataion Unit. 

On the Unit, they will receive therapy intervention and support around complex discharge planning, to enable a timely and safe discharge to their usual residence or to a more suitable location if appropriate.  We actively promote a multidisciplinary model on the Unit, where nursing staff work closely with therapists, social workers, GPs with Special Interest in Older people's Care as well as other Acute and Community colleagues, to ensure the person we are supporting is kept at the heart of every decision,

Main duties of the job

  • Responsible for responding to patient enquires either written, email face to face or telephone using empathy and judgment.
  • Processing referrals and all associated tasks within service guidelines and timescales.
  • Provide reception duties for building users at Skylark Rehabilitation Unit (and a meet and greet service for other building users).
  • Act in a professional manner as the 1st contact that patients/relatives will have with the service.  
  • Answer the telephone within a timely manner on all occasions.
  • Confirming and following up patient transport bookings.
  • Responsible for administration of discharging patients in accordance with local policy.
  • Responsible for preparing and filing patient records accurately-including paper and paperless formats.
  • Work on own initiative where appropriate and work alongside other teams to improve service delivery and patient experience.
  • Ensure own work and all outgoing material meets Sirona standards
  • Comply with and be actively involved in the development, implementation and review of new administration processes.
  • Takes initiative to organise won time on a day-to day basis in conjunction with colleagues and prioritises on-going work with guidance from line-manager to meet deadlines.
  • Escalates issues/risks as appropriate to ensure there is no impact on service delivery.

Skills & Experience

Essential criteria
  • Relevant experience in administrative work - managing patient appointments and bookings advantageous
  • Experience of dealing with patients/customers in a service environment including able to deal with routine and non-routine queries and progress
  • Experience of receiving calls from members of the public and dealing with queries/complaints
  • Experience of working in a team and an understand of team dynamics
  • Excellent organisational skills, able to forward plan
Desirable criteria
  • Knowledge of EMIS

Qualifications

Essential criteria
  • NVQ qualification in Information Technology/ECDL or equivalent experience/ other computer based subjects or willingness to undertake.
  • High degree of computer literacy
