As our Senior Discharge Coordinator, you will play a key role in supporting patient flow across our Bedded Rehabilitation Units, ensuring patients experience safe, timely and well-planned discharges.

Working closely with multidisciplinary teams, patients, families and partner organisations, you will lead proactive discharge planning from the point of admission. By identifying potential barriers early, coordinating services and maintaining clear communication, you will help reduce delays, improve patient flow and support seamless transitions of care.

You will be the central point of contact for discharge-related enquiries, coordinating with both internal teams and external partners to ensure discharge plans are delivered safely, efficiently and in line with each patient's individual needs. Your work will directly support the reduction of length of stay and the development of robust, efficient discharge processes for our Units.

While primarily based within one rehabilitation unit, you will be part of a supportive team of Discharge Coordinators working across five services on four sites. Reporting to the Rehabilitation Units' Flow Lead, you will work collaboratively with:

  • Unit Managers
  • Multidisciplinary Teams (MDTs)
  • Fellow Discharge Coordinators
  • The Rehabilitation Units' Flow Lead as part of a dedicated discharge team

Main duties of the job

  • To carry out initial meet and greet discussions upon arrival to the rehabilitation unit, outline expectations in regard to discharge planning and proactively manage any foreseen concerns or barriers to discharge.

  • To broker and chair initial future planning and discharge planning meetings, this will involve liaison with patient, relatives / carers and key members of the MDT involved in the patient’s care. Including the requirement for effective documentation and written follow up if needed.

  • Ensuring patients and carers are aware of the discharge process and the date and criteria the team are working towards for discharge.

  • To be part of the MDT at board rounds and Multi-disciplinary team meetings, understanding where the patient is in their pathway and actioning patient tracking tasks.

  • Working across multi- professional disciplines and assist to make referrals to social care, Reablement, and Pathway 1, ICSS Continuing Health Care (CHC) complex and CHC Fast Track (FT) referrals

Essential criteria
  • NVQ level 3 in Health or Business administration
  • Ability to design spreadsheets and input and extract data from Excel.
  • Educated to GCSE standard to include English.

Skills, Knowledge and Experience

Essential criteria
  • Experience working in a clinical envrionment
  • Knowledge of patient administrative systems, Units Bed flow and other Rehab Units Flow processes eg Careflow connect.
  • IT skills, confidence with Microsoft word, emails, keyboard skills, data collection including creation of spreadsheets using Microsoft excel.
  • Ability to collect, cleanse and present data for reporting purposes.
Desirable criteria
  • Minute/Note taking skills
  • Experience working with EMIS.
  • To have a good working understanding of matters related to the social care framework, care act assessments and hospital discharge processes.
