You will be a core member of the acute learning disability and autism liaison team, employed by Sirona, working within an acute trust.
Delivering both clinical and non-clinical support for patients as well as supporting the registered members of staff to deliver high quality care.
This is a busy role, covering both inpatient and outpatient areas. Supporting patients with learning disabilities and autistic patients to access healthcare through planning, reasonable adjustments, distraction and engagement.
You will be actively involved in developing and completing care plans and health passports for patients, based on their individual needs.
Direct patient support at outpatient appointments as well as on the inpatient wards and ED areas, where appropriate.
When appropriate, supporting hospital staff with personal care, meal times and mobilising patients.

- Working directly with patients with a learning disability.
- Working directly with autistic patients.
- Collaboratively working with patients, the team and the wider hospital to achieve holistic outcomes.
- Supporting the acute learning disability and autism liaison team, raising concerns and challenging inappropriate behaviour where appropriate.
- Communicating with a diverse group of patients, utilising various communication tools, including non-verbal resources.
- Supporting hospital staff to engage with patients in their treatment plans as well as activities of daily living.
- Supporting the team and hospital learning disability and autism steering group with improvement projects, audits and training.
- Developing therapeutic activities for patients.
- Managing own daily workload, with guidance from senior members of the team and clinical lead.
- Maintaining confidentiality, keeping accurate records on both paper and electronic notes.
- Supporting admin tasks where required.
- Supporting development of champion roles within the hospital.
- Supporting wider liaison team at other hospital sites, if required.
