Job overview

The In-patient Therapy service provides a 7-day service aiming to provide therapeutic input to maximise their functional ability and therefore work towards service user rehabilitation goals. When rehabilitation potential has been optimised the team help to organise a safe discharge, in most cases to the patient’s home, with advice and support for the patient and family/carers.

We actively promote a multi-disciplinary model, where occupational therapy staff work closely with physiotherapists, nursing staff, social workers and support staff; ensuring an ‘enablement’ ethos and that the patients are kept at the centre of the decision making.

We require a band 6 Occupational Therapist to work and support the therapy team in providing assessments for care planning to enable the discharge of the patients on the unit to an appropriate destination in a timely manner. The therapy team consists of physiotherapists, assistant practitioners and rehab support workers.

Main duties of the job

  • Responsible for the initial holistic assessment, implementation and evaluation of programmes of care for service users who are acutely and chronically unwell; often with complex needs arising from the frail elderly cohort
  • Use clinical reasoning skills to assess, plan, implement and evaluate, person centred intervention programmes incorporating physical, psychological, social, leisure and employment needs, using evidence-based practice approaches.
  • Collaborate and negotiate with service users, carers and family in identifying appropriate and achievable goals as part of the overall care plan, which will maximise quality of life and independence.
  • Facilitate appropriate referrals to other services or investigations based on your assessment to ensure holistic patient care
  • Work within a multi-disciplinary team, to provide integrated skills and sharing knowledge providing holistic and specialist skills supported by a robust competency framework

Essential criteria
  • HCPC registration
  • BSc degree in Occupational therapy
  • Full Driving Licence and access to car in work time (with business use car insurance)

Skills, Knowledge and Experience

Essential criteria
  • Ability to supervise Band 5 therapists, support workers and therapy students effectively when required and give guidance and leadership to all within the MDT
  • To have gained effective experience of rehabilitation in relevant fields as an Occupational Therapist
  • To be competent in performing highly skilled assessments and treatments with a variety of presenting conditions, including those associated with frail elderly patients
Desirable criteria
  • Recent relevant experience within the community and acute settings including the management of the frail complex patient
