Job overview
This exciting role is for a physiotherapist looking to develop their specialist skills further in Stroke Rehabilitation.
The post is in the Sub-Acute Stroke Service based in South Bristol Community Hospital. The unit consists of 30 beds which provides an environment where individuals who are medically stable can receive rehabilitation assessment, along with focused therapy and support from our multidisciplinary team, to develop independence before they return home
You will be an autonomous practitioner with proven stroke therapy skills in caseload management and specialist knowledge of stroke rehabilitation.
You will be working with an experienced Multidisciplinary team, providing lots of scope for training and development and work closely with the our community colleagues in the Integrated Community Stroke Team.

-To assess, plan, implement and monitor care, facilitating best practice and evidence-based, person-centred care
- To accept clinical responsibility for a caseload of stroke patients with
complex/highly complex needs
- To work within defined specialist, department and national protocols/policies for stroke care such as the NICE clinical guidelines and National Stroke Strategy.
- To be actively involved in monitoring and developing the Stroke service, to ensure patients receive sustainable high quality care that is effective and safe

- Degree/diploma in Physiotherapy
- HCPC registration
Knowledge

- Evidence of continuing professional development in stroke care
- Wide range of CPD including neurology
Desirable criteria
- Service development or audit experience
Experience
Essential criteria
- Experience in assessment and management of people after stroke as part of a MDT
- Experience in supervision of more junior staff/students
- Experience of complex discharge planning
- Experience complex communication skills (patients / carers)
