Job overview

An exciting opportunity has arisen within Pathway 3 for a motivated and compassionate Rehabilitation Support Worker with an interest in supporting the assessment of individuals following hospital discharge.

The successful candidate will be confident working independently as well as within a team. You will possess excellent communication skills, demonstrate initiative and contribute to ensuring service users and their families receive a positive experience.

You will work alongside Physiotherapists to assess service users who have been discharged from hospital into assessment beds within local nursing homes. You will assist with gathering information, supporting prescribed activities and reporting relevant findings.

The role offers an opportunity to work closely with individuals during their assessment journey. You will help clinicians build a comprehensive picture of a person's needs and potential support requirements, contributing to informed decisions about future care and discharge planning.

You will be part of a friendly and supportive team committed to delivering high-quality, person-centred care. Regular training, supervision and opportunities for professional development will be provided to help you develop your knowledge and skills.

The post is based within nursing homes throughout South Glos and requires flexibility to work across multiple sites.

Main duties of the job

·       Promote optimum independence through encouraging rehabilitation techniques.

·       As a rehab support worker you will be a key figure in completing the day to day therapy input with patients under the instruction of the lead therapists. You will be required to provide feedback on the service users’ progress and help to ensure that the service users achieve their maximum potential.

·       Undertake a range of delegated clinical and therapy led activities to support the service users on the caseload.

·       To provide safe, patient centred, effective and evidence-based clinical activities.

·       The post holder will work under direct supervision and be responsible to senior therapists providing one to one and group rehabilitation.

·       You will be part of an integrated multi-disciplinary team (MDT), working with partner organisations which includes nurses, physiotherapist, occupational therapists and Social Workers.

The Service operational Hours are between 08:00 and 18:00 Monday- Friday (flexible working can be discussed) with future possibility for weekend cover.

Essential criteria
  • NVQ level 3 in Health & Social Care or willingness to work towards
  • Numeracy and literacy level 2 or equivalent
  • Full driving licence and access to car in work time
Desirable criteria
  • Apprenticeship in Health (level 3)

Skills, Knowledge & Experience

Essential criteria
  • Able to work as part of a multidisciplinary team but also to act on own initiative
  • Effective communication and interpersonal skills
  • Experience of direct patient care in a health or social care setting
  • Experience working without direct supervision
  • Able to give examples of ability to prioritise tasks
Desirable criteria
  • Recent relevant experience within the community setting
