Are you looking for an opportunity to play a pivotal role in Betsi Cadwaladr University Health Board’s, first point of contact for patients services?
As a Single Point of Contact (SPOC) and Self Care Service Facilitator, you will provide a high level of service to the Single Point of Contact team, which forms the basis of the 3P’s delivery.

Main duties of the job
An exciting opportunity has been created for a self management facilitator to join the Self Care Team in BCU. This role is fixed term and will involve supporting patients as a Health Coach and facilitating the delivery of Self management education specifically to support the Health and Wellbeing courses. Predominantly working from home via a teams enabled laptop.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
